清胰汤联合生长抑素对高脂血症性胰腺炎患者胃肠与凝血功能及免疫系统影响

Effect on gastrointestinal and coagulation function and immune system in the patients with hyperlipidemia pancreatitis treated with qingyi decoction and somatostatin

目的分析清胰汤联合生长抑素对高脂血症性胰腺炎(HLP)患者胃肠与凝血功能及免疫系统影响。方法选取2017年1月至2019年1月间接受治疗的HLP患者114例,随机数字表法分成两组,对照组57例采用生长抑素治疗,治疗组57例在对照组基础上加服清胰汤。观察两组患者临床疗效,患者住院时间、腹痛消失时间、发生全身多器官功能衰竭(MODS)概率及住院期间死亡率情况,检测治疗前后血清凝血功能、免疫功能、胃肠功能等血清指标改变情况。结果治疗后治疗组总有效率为94.74%、对照组为77.19%,两组比较差异有统计学意义(P<0.05)。治疗组患者住院时间、腹痛消失时间、MODS发生率均低于对照组,差异有统计学意义(P<0.05)。两组患者治疗后全血低切黏度、全血高切黏度、血浆黏度、IL-6、IL-8及TG含量较治疗前降低(P<0.05);且治疗组优于对照组(P<0.05)。两组患者治疗后血清CD4+、Lym含量较治疗前升高,CD8+含量较治疗前降低(P<0.05);且治疗组治疗后血清CD4+、Lym含量升高优于对照组,CD8+含量降低优于对照组(P<0.05)。两组患者治疗后血清MTL含量较治疗前升高,GIP、VIP含量较治疗前降低(P<0.05);且治疗后治疗组血清MTL含量升高优于对照组,GIP、VIP含量降低优于对照组(P<0.05)。结论清胰汤联合生长抑素对HLP患者疗效显著,可改善其血液高凝状态、胃肠功能,降低血清炎性因子含量,提升细胞免疫功能,改善机体微循环,促进胰腺功能恢复。

Objective To analyze the effect on gastrointestinal and coagulation function and immune system in the patients with hyperlipidemia pancreatitis(HLP)treated with qingyi decoction and somatostatin.Methods From January 2017 through to January 2019,114 patients with HLP were divided into two groups according to random number table.In the control group,57 cases were treated with somatostatin.In the treatment group,57 cases were treated with qingyi decoction on the base of the treatment as the control group.The clinical effect,hospitalization time,disappearance time of abdominal pain,the probability of multiple organ dysfunction syndrome(MODS)and the in-hospital mortality were observed in the two groups.Before and after treatment,the serum coagulation function,immune function and gastroinestinal function were tested.Results After treatment,the total effective rate was 94.74%in the treatment group and was 77.19%in the control group,indicating the statistical difference(P<0.05).The hospitalization time,the disappearance time of abdominal pain and the incidence of MODS in the treatment group were lower than the control group,indicating the statistical significance(P<0.05).After treatment,the whole blood low shear viscosity,high shear viscosity,plasma viscosity,IL-6,IL-8 and TG were all lower than those before treatment in the two groups(P<0.05).The results of those items in the treatment group were better than the control group(P<0.05).After treatment,the levels of CD4+and Lym were increased as compared with those before treatment and CD8+was reduced in the two groups(P<0.05).The increase of serum CD4+and Lym after treatment in the treatment group was better than the control group and the decrease of CD8+was better than the control group(P<0.05).Serum MTL after treatment was increased and GIP and VIP reduced as compared with those before treatment in the two groups(P<0.05).The increase of MTL and the decrease of GIP and VIP in the treatment group were better than the control group(P<0.05).Conclusion The combined medication of qingyi decoction and somatostatin achieves the significant effect on HLP.It improves in blood coagulation state and gastrointestinal function,reduces serum inflammatory factor level and promotes cell immune function,microcircualtion and the recovery of pancreas function.
