四步按摩法联合多药物治疗血液透析患者动静脉内瘘血栓的临床研究

Clinical Study of Four-step Massage Combined with Multi-drug Therapy for Arteriovenous Fistula in Hemodialysis Patients

目的探究对血液透析患者动静脉内瘘血栓应用四步按摩法与多药物联合治疗的效果。方法临床方便选取2017年1月—2018年12月该院接收的动静脉内瘘血栓血液透析患者共42例,采取数字表法随机分成两组各21例,对照组应用多药物治疗,观察组在前者基础上加以四步按摩法,对两组疗效进行观察。结果观察组内瘘再通率是80.95%明显较对照组的52.38%高(χ^2=18.365,P<0.05),局部血肿率是33.33%明显较对照组的57.14%低(χ^2=11.442,P<0.05),再通后的首次透析血流量是(232.47±21.35)mL/L明显较对照组的(218.25±16.38)mL/L多(t=4.942,P<0.05),溶通时间是(16.12±5.65)min明显较对照组的(21.68±6.38)min短(t=3.633,P<0.05),差异有统计学意义(P<0.05)。结论对血液透析患者动静脉内瘘血栓采取四步按摩法与多药物联合治疗效果显著,成功率较高,再通较快,血肿出现较少,值得广泛采用。

Objective To investigate the effect of four-step massage and multi-drug combination therapy on arteriovenous fistula in hemodialysis patients. Methods A total of 42 patients with arteriovenous fistula hemodialysis were convenient enrolled in our hospital from January 2017 to December 2018. They were randomly divided into two groups according to the digital table method. The control group was treated with multi-drug therapy. The observation group was given four steps massage method on the basis of the former, the two groups of effects were observed. Results In the observation group, the recanalization rate was 80.95%, which was significantly higher than that of the control group (χ^2=18.365, P<0.05). The local hematoma rate was 33.33%, which was significantly lower than the control group's 57.14% (χ^2=11.442, P<0.05), the first dialysis blood flow after recanalization was (232.47±21.35)mL/L significantly more than the control group (218.25±16.38)mL/L (t=4.942, P<0.05), and the dissolution time was (16.12±5.65) min was significantly shorter than the control group (21.68±6.38)min(t=3.633, P<0.05), the difference was significant(P<0.05). Conclusion The four-step massage method combined with multi-drug therapy for hemodialysis patients has a significant effect. The success rate is higher, the recanalization is faster, and the hematoma is less. It is worthy of widespread adoption.
