摘要
目的探讨阑尾粘液性肿瘤的多层螺旋CT表现及其诊断价值。方法收集和分析2010年3月—2014年12月18例经病理证实的阑尾粘液性肿瘤,其中阑尾粘液腺癌8例,阑尾粘液性囊腺瘤4例,粘液性囊肿6例。结果CT对阑尾粘液性疾病的诊断准确率为90%,与手术病理诊断差异无统计学意义(P>0.05)。8例粘液性囊腺癌表现为阑尾不均匀增粗>1 cm,壁不规则增厚,增强壁、分隔不均匀强化,其中6例见斑点、环状钙化。4例粘液性囊腺瘤表现为阑尾增粗>1.5 cm,壁稍增厚且强化,其中2例见斑片状钙化,2例呈囊实性改变。6例粘液性囊肿表现阑尾囊袋状扩张>3cm,壁薄且光滑,增强未见明显强化,其中2例囊壁边缘见条状钙化。结论MSCT对阑尾粘液性肿瘤的诊断和鉴别诊断有较大价值。
Objective To investigate the multi-slice spiral CT findings of the appendix mucinous tumor and its diagnostic value. Methods 18 cases of pathologically confirmed appendix mucinous tumors from March 2010 to December 2014 were collected and analyzed, including 8 cases of appendix mucinous adenocarcinoma, 4 cases of appendix mucinous cystadenoma, and 6 cases of mucinous cyst. Results The diagnostic accuracy of CT for appendical mucinous disease was 90%, and there was no significant difference between the two groups (P>0.05). 8 cases of mucinous cystadenocarcinoma showed uneven thickening of the appendix >1 cm, irregular wall thickening, enhanced wall and uneven separation enhancement, and 6 cases showed spot and annular calcification. 4 cases of mucinous cystadenoma showed thickening of the appendix >1.5cm, and the wall was slightly thickened and strengthened. Among them, 2 cases showed patchy calcification and 2 cases showed cystic changes. 6 cases of mucinous cysts showed a dilatation of the appendix sac > 3 cm, the wall was thin and smooth, and no enhancement was observed in the enhancement. 2 cases of the cystic wall margins showed strip calcification. Conclusion MSCT has great value in the diagnosis and differential diagnosis of appendix mucinous tumors.
