补肾活血祛浊法治疗慢性肾衰竭并发急性痛风临床观察

Clinical Observation on Tonifying the Kidney and Activating Blood Circulation for Removing Turbid Therapy in Treating Chronic Renal Failure Complicated with Acute Gout

目的观察补肾活血祛浊中药内外合治慢性肾衰竭并发急性痛风的临床疗效。方法将60例患者随机分2组,每组各30例。治疗组内服加味清血汤、外用止痛贴,对照组口服美洛昔康,治疗15 d后统计疗效。结果治疗组治疗后血肌酐(Scr)、血尿素氮(BUN)、血尿酸(SUA)、C反应蛋白(CRP)、血沉(ESR)降低,肾小球滤过率(GFR)升高,中医证候积分及痛风证候积分减少,与治疗前相比差异有统计学意义。对照组治疗后CRP、ESR降低,痛风证候积分减少,与治疗前相比差异有统计学意义。2组治疗后比较,治疗组BUN、SUA、CRP、ESR降低,中医证候积分及痛风证候积分减少,与对照组相比差异有统计学意义。治疗组的痛风证候缓解时间较对照组减少,差异有统计学意义。结论补肾活血祛浊中药内外合治慢性肾衰竭并发急性痛风有疗效。

Objective To observe clinical curative effect of tonifying the kidney and activating blood circulation for removing turbid therapy in treating chronic renal failure complicated with acute gout using medicine orally and externally together. Methods 60 patients were randomly divided into two groups, with 30 cases in each group. The treatment group was treated with modified Qingxue decoction orally and Zhitongtie externally, while the control group was treated with Meloxicam tablet orally. The curative effect was counted after 15 days of treatment. Results In the treatment group after treatingt, Scr, BUN, SUA, CRP and ESR decreased, GFR increased, TCM syndrome integral and gout syndrome integral decreased, and there was significant difference compared with before treatingt. After treatment, CRP and ESR decreased and gout syndrome scores decreased in the control group, which had significant difference compared with those before treatment. Compared with the control group, BUN, SUA, CRP and ESR in the treatment group decreased, while the scores of TCM syndromes and gout syndromes decreased. There was significant difference between the two groups. The relief time of gout syndrome in the treatment group was less than that in the control group, and the difference was statistically significant. Conclusion It is effective to treat chronic renal failure complicated with acute gout by tonifying the kidney and activating blood circulation for removing turbid therapy.
