盐酸右美托咪定应用于超声引导下臂丛神经阻滞的镇痛效果分析 被引量:4

摘要 目的观察盐酸右美托咪定用于超声引导下臂丛神经阻滞的镇痛效果。方法将2016年3月—2017年3月在该院接受治疗的80例前臂或手部手术患者随机分为实验组(n=40)和对照组(n=40),两组所有患者均在超声引导下进行臂丛神经阻滞。对照组予以予0.375%左布比卡因+生理盐水,实验组予以0.375%左布比卡因+右美托咪定。分别比较两组感觉阻滞起效时间、感觉阻滞持续时间以及镇痛时间。结果实验组患者感觉阻滞起效时间短于对照组[(8.56±1.74)min vs(11.73±2.19)min],而感觉阻滞持续时间及镇痛持续时间则长于对照组[持续时间:(652.03±53.61)min vs(430.08±42.83)min,镇痛:(762.70±68.90)min vs(555.98±74.88)min],上述差异有统计学意义(t=7.168、20.457、12.849,P<0.001)。结论盐酸右美托咪定能够显著缩短局部麻药的起效时间,延长感觉阻滞持续时间及镇痛时间。 Objective To observe the analgesic effect of Dexmedetomidine on ultrasound-guided brachial plexus block.Methods The 80 patients with forearm or hand surgery treated in our hospital in March 2016~2017 March were randomly divided into experimental group(n=40)and control group(n=40).Brachial plexus block was performed under ultrasound guidance in two groups.The control group was given 0.375%Levobupivacaine+saline,and the experimental group was given 0.375%Dexmedetomidine+Levobupivacaine.The effective time of sensory block,the duration of sensory block and the analgesic time were compared between two groups.Results The onset time of sensory block in the experimental group was shorter than that in the control group[(8.56±1.74)min vs(11.73±2.19)min],while the duration of sensory block and the duration of postoperative analgesia in the experimental group were longer than that of control group[duration:(652.03±53.61)min vs(430.08±42.83)min,analgesia:(762.70±68.90)min vs(555.98±74.88)min].The above differences were statistically significant(t=7.168,20.457,12.849,P<0.001).Conclusion Dexmedetomidine can significantly shorten the effective time of local anesthesia,prolong the duration of sensory block and postoperative analgesia time.
