炎性细胞因子在溃疡性结肠炎患者中的表达及其对预后的影响

Expression of Inflammatory Cytokines in Patients with Ulcerative Colitis and Its Effect on Prognosis

目的分析炎性细胞因子在溃疡性结肠炎患者中的表达及其对预后的影响。方法在2016年7月—2018年5月开展该次研究,期间总共选取活动期UC患者25例作为案例进行研究。另外选取通过规范治疗且治疗3个月后复查结果理想的活动期UC患者25例作为对照患者。两组患者均接受免疫组化技术进行检查。总结并对比两组患者的肠道黏膜表达以及光密度数据。结果不同时期UC患者均存在一定程度的炎性细胞因子阳性表达,同时呈现出逐渐性的减弱,IL-17、IL-23、IL-22、IL-11的表达方式主要是体现在结肠上皮细胞以及固有层内单个细胞胞质方面;活动期患者IL-17、IL-23、IL-22、IL-11这些炎性因子的表达量(0.072 5±0.003 6);(0.140 5±0.005 8);(0.051 3±0.004 4);(0.025 1±0.001 3)均显著高于对照组患者(0.034 4±0.024 3);(0.075 4±0.004 0);(0.0260±0.007 1);(0.036 6±0.002 5),表达差异突出,组间对比结果有意义(IL-17对比结果(t=6.587,P=0.023)、IL-23对比结果(t=7.214,P=0.031)、IL-22对比结果(t=6.994,P=0.020)、IL-11对比结果(t=6.889 5,P=0.031)。结论炎性细胞因子在溃疡性结肠炎患者中的表达有着较高特殊性,可以准确评估患者的病情状况,同时表达水平对于短期的治疗效果以及治疗后的恢复情况有一定预见测评价值,值得临床推广。

Objective To analyze the expression of inflammatory cytokines in patients with ulcerative colitis and its effect on prognosis.Methods The study was conducted from July 2016 to May 2018.A total of 25 UC patients in active phase were selected.In addition,25 active UC patients who received standard treatment and had ideal results after 3 months of treatment were selected as the control group.Both groups were examined by immunohistochemical technique.The intestinal mucosal expression and optical density data of the two groups were summarized and compared.Results There were some positive expressions of inflammatory cytokines in UC patients at different stages,and there was a gradual decrease.The expression patterns of IL-17,IL-23,IL-22 and IL-11 were mainly expressed in colonic epithelial cells and single cell cytoplasm within the lamina propria.The expression levels of IL-17,IL-23,IL-22 and IL-11 in the active patients(0.072 5±0.003 6);(0.140 5±0.005 8);(0.051 3±0.004 4);(0.025 1±0.001 3)were significantly higher than those in the control group(0.034 4±0.024 3);(0.075 4±0.004 0);(0.026 0±0.007 1);(0.036 6±0.002 5),with statistically significant differences between the two groups(t=6.587,7.214,6.994,6.889,P=0.023,0.031,0.020,0.031,P<0.05).Conclusion The expression of inflammatory cytokines in patients with ulcerative colitis has a high specificity.It can evaluate the severity of the disease to a certain extent.And the expression level and prognostic effect of mucosal healing after short-term treatment have certain reference value,which is worthy of clinical promotion.
