老年慢性阻塞性肺疾病合并重症呼吸衰竭无创通气治疗的护理的效果分析

Nursing Effect Analysis of Non-Invasive Ventilation in Elderly Patients with Chronic Obstructive Pulmonary Disease Complicated with Severe Respiratory Failure

目的探讨老年慢性阻塞性肺疾病合并重症呼吸衰竭无创通气治疗的护理的效果。方法选取该院2017年3月—2018年5月收治的48例老年慢性阻塞性肺疾病合并重症呼吸衰竭患者,采取无创通气治疗,同时予以精心护理,对比该组患者治疗前后RR、HR、GCS、辅助呼吸肌评分及动脉血气分析指标。结果该组患者治疗后RR、HR、GCS、辅助呼吸肌评分较治疗前差有统计学意义(t=8.89、13.85、29.30、13.17,P<0.05);该组患者中,完全依从29例(60.4%),部分依从16例(33.3%),不依从3例(6.25%),依从率93.75%;该组患者中,非常满意30例(62.5%),满意16例(33.3%),不满意2例(4.2%),总满意率95.8%。结论无创通气治疗慢性阻塞性肺疾病操作简单、创伤小,同时治疗期间采取优质护理,可增强患者护理依从性,可进一步改善患者通气效果,促使患者早日康复,提高患者的满意度。

Objective To explore the nursing effect of noninvasive ventilation for elderly patients with chronic obstructive pulmonary disease complicated with severe respiratory failure.Methods A total of 48 cases of elderly patients with chronic obstructive pulmonary disease complicated with severe respiratory failure admitted to our hospital from July 2017 to May 2018 were selected and treated with noninvasive ventilation and meticulous nursing care.RR,HR,GCS,auxiliary respiratory muscle score and arterial blood gas analysis indexes were compared before and after treatment in this group of patients.Results After treatment,the RR,HR,GCS,and auxiliary respiratory muscle scores of the patients in this group were statistically significant difference from those before treatment(t=8.89,13.85,29.30,13.17,P<0.05).Among the patients in this group,29 cases(60.4%)were fully compliant,16 cases(33.3%)were partially compliant,and 3 cases(6.25%)were noncompliant,with a compliance rate of 93.75%.Among the patients in this group,30 cases(62.5%)were very satisfied,16 cases(33.3%)were satisfied,and 2 cases(4.2%)were dissatisfied,with a total satisfaction rate of 95.8%.Conclusion Noninvasive ventilation in the treatment of chronic obstructive pulmonary disease is simple in operation and less traumatic.Meanwhile,high-quality nursing during the treatment can enhance the patient's nursing compliance,further improve the patient's ventilation effect,promote the patient's early recovery,and improve the patient's satisfaction.
