成人早期股骨头坏死应用CT和MRI检查诊断的临床价值 被引量:2

摘要 目的探讨电子计算机断层扫描(CT)和磁共振成像(MRI)检查在成人早期股骨头坏死诊断中的价值。方法选取2017年10月至2018年12月收治的80例(98髋)疑似早期股骨头坏死成年患者,入院后均行CT、MRI诊断,检查后均随访记录其手术诊断结果。以手术诊断结果为金标准,评估CT与MRI检测成人早期股骨头坏死准确度、灵敏度、特异度;并对比CT与MRI对不同时期股骨头坏死诊断情况。结果80例(98髋)患者经手术病理证实,70髋为股骨头坏死,临床分期:Ⅰ期股骨头坏死16髋,Ⅱ期股骨头坏死19髋,Ⅲ期股骨头坏死20髋,Ⅳ期股骨头坏死15髋。MRI诊断成人早期股骨头坏死准确度(91.84%)、灵敏度(92.86%)、特异度(89.29%)均高于CT诊断(74.49%、78.57%、64.29%),差异均有统计学意义(P<0.05);MRI诊断Ⅰ~Ⅱ期股骨头坏死准确率(88.57%)、总准确率(92.86%)高于CT诊断(65.71%、78.57%),差异均有统计学意义(P<0.05);MRI诊断Ⅲ、Ⅳ期股骨头坏死准确率(95.00%、100.00%)与CT诊断(90.00%、93.33%)比较,差异无统计学意义(P>0.05)。结论相较于CT诊断,成人早期股骨头坏死采用MRI检测可提升诊断准确度、灵敏度、特异度,为临床诊治疾病提供重要依据。
