SIRT2在非小细胞肺癌组织中的表达及临床意义

摘要 目的探讨沉默信息调节因子2(SIRT2)蛋白在非小细胞肺癌(NSCLC)组织中的表达及与临床病理特征和预后的关系。方法采用免疫组化法检测2011年1月至2013年12月243例NSCLC组织及120例癌旁组织中SIRT2的表达,分析其表达水平与临床病理特征的关系,采用Cox风险比例回归模型分析影响预后的因素。结果SIRT2在NSCLC组织中的高表达率为31.3%(76/243),低于癌旁组织的57.5%(69/120),差异有统计学意义(P<0.001)。SIRT2表达与分化程度和淋巴结转移有关(P<0.05),而与年龄、性别、肿瘤大小和TNM分期均无关(P>0.05)。SIRT2高表达患者的中位无病生存期(DFS)为37.0个月(95%CI:25.3~48.7个月),长于低表达者的31.0个月(95%CI:27.3~34.8个月),差异有统计学意义(P=0.026);SIRT2高表达患者的中位总生存期(OS)为54.0个月(95%CI:40.7~67.3个月),长于低表达者的41.0个月(95%CI:33.6~48.5个月),差异有统计学意义(P=0.023)。Cox多因素回归分析结果显示,分化程度、淋巴结转移是影响NSCLC患者DFS和OS的独立因素(P<0.05),而SIRT2表达对NSCLC患者的预后无独立预测作用(P>0.05)。结论SIRT2在NSCLC组织中低表达,其高表达与NSCLC更好的临床病理特征及预后有关。 Objective To detect the expression of silent information regulator 2(SIRT2)in non-small cell lung cancer(NSCLC),and to explore its correlation with the clinicopathological charcteristics and prognosis in NSCLC patients.Methods The expression of SIRT2 in 243 NSCLC tissues and 120 adjacent tissues from Jan 2011 to Dec 2013 was detected by immunohistochemical staining.The relationship between SIRT2 expression level and clinicopathological charcteristics was analyzed.Cox risk proportional regression model was used to analyze the prognostic factors.Results The high expression rate of SIRT2 in NSCLC tissues was 31.3%(76/243),lower than 57.5%(69/120)in adjacent tissues(P<0.001).The expression of SIRT2 in NSCLC tissues was correlated with the degree of differentiation and lymph node metastasis(P<0.05),but not with age,sex,tumor size and TNM stage(P>0.05).The median disease-free survival(DFS)of patients with high SIRT2 expression was 37.0 months(95%CI:25.3-48.7 months),longer than 31.0 months(95%CI:27.3-34.8 months)of patients with low SIRT2 expression(P=0.026).The median overall survival(OS)of patients with high SIRT2 expression was 54.0 months(95%CI:40.7-67.3 months),longer than 41.0 months(95%CI:33.6-48.5 months)of patients with low SIRT2 expression(P=0.023).Multivariate Cox regression analysis showed that pathological grade and lymph node metastasis were independent prognostic factors for DFS and OS in NSCLC patients(P<0.05),while SIRT2 expression level had no independent predictive effect on DFS and OS in NSCLC patients(P>0.05).Conclusion SIRT2 is low expressed in NSCLC tissues compared to paired adjacent tissues,and its high expression is correlated with better clinicopathological characteristics and prognosis in NSCLC patients.
