两种手术方法治疗胫骨远端骨折的疗效对比

Comparison of two surgical methods in the treatment of distal tibia fracture

目的:比较用胫骨多向锁定带锁髓内针内固定术与微创经皮胫骨远端钢板内固定术治疗胫骨远端骨折的效果。方法:选取2014年3月至2019年3月期间四川省叙永县人民医院收治的120例胫骨远端骨折患者进行分组研究。将其分为对照组(60例)和观察组(60例)。为对照组患者应用微创经皮胫骨远端钢板内固定术进行治疗,为观察组患者应用胫骨多向锁定带锁髓内针内固定术进行治疗。然后比较两组患者术中的出血量、手术的时间、治疗的优良率和术后并发症的发生情况。结果:观察组患者治疗的优良率(91.67%)高于对照组患者治疗的优良率(73.33%),P<0.05;观察组患者术后并发症的发生率(8.33%)低于对照组患者术后并发症的发生率(28.33%),P<0.05;与对照组患者相比,观察组患者术中的出血量较少,其手术的时间较短,P<0.05。结论:与采用微创经皮胫骨远端钢板内固定术相比,用胫骨多向锁定带锁髓内针内固定术治疗胫骨远端骨折的效果较好,可有效地减少患者术中的出血量,缩短其手术的时间,且安全性较高。

objective:to compare the efficacy of tibial multidirectional locking with intramedullary needle internal fixation and minimally invasive percutaneous internal fixation for distal tibial fractures.Methods:a total of 120 patients with distal tibia fractures admitted to the people’s hospital of xuyong county,sichuan province from March 2014 to March 2019 were studied in groups.They were divided into control group(60 cases)and observation group(60 cases).Patients in the control group were treated with minimally invasive percutaneous tibial distal plate internal fixation,and patients in the observation group were treated with tibial multidirectional locking with locking intramedullary needle internal fixation.Then,the intraoperative blood loss,operation time,excellent and good rate of treatment and postoperative complications were compared between the two groups.Results:the excellent and good rate(91.67%)of the observation group was higher than that of the control group(73.33%),P<0.05.The incidence of postoperative complications in the observation group(8.33%)was lower than that in the control group(28.33%),P<0.05.Compared with the control group,the observation group had less intraoperative blood loss and shorter operative time(P<0.05).Conclusion:compared with minimally invasive percutaneous plate internal fixation of the distal tibia,internal fixation with tibial multi-directional locking and locking intramedullary needle has a better effect on the treatment of the distal tibial fracture,which can effec tively reduce the amount of intraoperative blood loss,shorten the operation time,and has a higher safety.
