对接受维持性血液透析的糖尿病肾病患者使用TDI-Tei指数评价其右心功能的效果分析

Analysis of the effects of TDI-Tei index on right cardiac function in patients with diabetic nephropathy receiving maintenance hemodialysis

目的:分析对接受维持性血液透析(MHD)的糖尿病肾病(DN)患者使用组织多普勒成像(TDI)-心肌做功指数(Tei指数)评价其右心功能的效果。方法:选择2018年1月至2019年3月期间在惠州市第三人民医院接受MHD的58例DN患者与58例单纯患有糖尿病(DM)的患者作为研究对象。将其中58例接受MHD的DN患者设为观察组,将其中58例单纯患有DM的患者设为对照组。对两组患者均进行超声检查和右心功能检查,比较其TDI-Tei指数和右心功能的各项指标。结果:观察组患者的TDI-Tei指数大于对照组患者,P<0.05。观察组患者的RVIDd大于对照组患者,其RVEF低于对照组患者,其T-E/T-A的比值和T-e/T-a的比值均小于对照组患者,其T-A、T-E、T-e和T-a均慢于对照组患者,P<0.05。结论:接受MHD的DN患者其TDI-Tei指数越大,其右心功能越差。对此类患者的TDI-Tei指数进行检测有助于评价其右心功能。

objective: to analyze the effect of tissue doppler imaging(TDI)-myocardial work index(Tei index) on right cardiac function of diabetic nephropathy(DN) patients receiving maintenance hemodialysis(MHD). Methods: 58 DN patients and 58 patients with diabetes mellitus(DM) who received MHD in huizhou third people’s hospital from January 2018 to March 2019 were selected as research objects. Among them, 58 DN patients receiving MHD were set as the observation group, and 58 patients with DM alone were set as the control group. Ultrasound examination and right heart function examination were carried out in both groups to compare the tdi-tei index and various indicators of right heart function. Results: the tdi-tei index of the observation group was higher than that of the control group, P < 0.05. The RVIDd of patients in the observation group was higher than that of patients in the control group, their RVEF was lower than that of patients in the control group, and their t-e/t-a ratio and t-e/t-a ratio were lower than those of patients in the control group, and their t-a, t-e, t-e and t-a ratios were slower than those of patients in the control group, P < 0.05. Conclusion: the greater the TDI-Tei index, the worse the right cardiac function. The TDI-Tei index of these patients was helpful to evaluate the right cardiac function.
