血清CX3CL1、TXNIP和Vaspin表达水平与冠心病患者病变程度的关系 被引量:15

The Relationship between Levels of CX3CL1,TXNIP,Vaspin and Severity of Coronary Heart Disease

摘要 目的研究冠心病患者血清趋化因子fractalkine(CX3CL1)、硫氧还蛋白互作蛋白(thioredoxin interacting proteins,TXNIP)和内脏脂肪特异性丝氨酸蛋白酶抑制剂(visceral fat-specific serine protease inhibitors,Vaspin)表达水平与疾病病变程度的关系。方法选取在我院诊治的100例冠心病患者,以同期100例健康人群为对照组,分别检测两组患者入院后的血脂水平和血清Vaspin、CX3CL1、TXNIP,同时分析单独检测和联合检测对患者的不同病变程度预测效率。结果观察组患者的总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、尿酸(UA)、尿素氮(BUN)水平明显高于对照组,随着患者的Gensini积分的提升,患者的LDL明显上升,HDL明显下降(t=8.21,7.24,P<0.05);观察组患者血清CX3CL1、TXNIP水平显著高于对照组(t=7.89,5.32,P<0.05),Vaspin水平显著低于对照组(t=8.21,P<0.05),随着患者的Gensini积分的逐渐升高,患者的Vaspin水平显著下降,血清CX3CL1、TXNIP显著上升(P<0.05);观察组患者的CX3CL1、TXNIP水平与Gensini积分呈现正相关(r=0.65~0.77,P<0.01),Vaspin水平与Gensini积分呈现负相关(r=-0.78,P<0.01),血清CX3CL1、TXNIP和Vaspin水平均为冠心病患者严重程度的独立危险因素;通过ROC曲线分析,随着Gensini积分的逐渐升高,血清Vaspin、CX3CL1、TXNIP水平的临界值依次为1.88μg/L,299.35ng/m L,2144.12ng/m L;1.55μg/L,317.12ng/m L,2254.53ng/m L;0.79μg/L,418.12ng/m L,2335.12ng/m L。结论血清Vaspin、CX3CL1、TXNIP水平在冠心病发生发展的不同阶段表达水平不同,联合检测以上指标对冠心病患者的不同病变程度预测具有重要意义。 Objective To study the relationship between the levels of serum CX3 chemokine ligand 1(CX3 CL1),thioredoxin interacting proteins(TXNIP),visceral fat-specific serine protease inhibitors(Vaspin)and the degree of disease in patients with coronary heart disease.Methods 100 patients with coronary heart disease treated in our hospital were selected,and 100 healthy people were enrolled as control group.The blood lipid levels and serum Vaspin,CX3 CL1 and TXNIP were measured.The prediction efficiency of individual and joint testing was analyzed.Results The levels of Total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL),high density lipoprotein(HDL),uric acid(UA)and urea nitrogen(BUN)in the observation group were significantly higher than those in the control group(P<0.05).With the increase of Gensini score,LDL increased and HDL decreased significantly(P<0.05).The serum levels of CX3 CL1 and TXNIP in the observation group were significantly higher than those in control group(P<0.05).The level of serum Vaspin in observation group was significantly lower than that in control group(P<0.05).With the gradual increase of Gensini scores,the level of Vaspin in the observation group decreased significantly,while the levels of serum CX3 CL1 and TXNIP increased significantly(P<0.05);CX3 CL1 and TXNIP levels in the observation group were positively correlated with Gensini scores,Vaspin levels were negatively correlated with Gensini scores,and serum CX3 CL1,TXNIP and Vaspin levels were independent risk factors for the severity of coronary heart disease;ROC curve analysis showed that with the gradual increase of Gensini scores,blood levels were independent risk factors for the severity of coronary heart disease.The critical values of clearing Vaspin,CX3 CL1 and TXNIP were 1.88 ug/L,299.35 ng/m L,2144.12 ng/m L,1.55 ug/L,317.12 ng/m L,2254.53 ng/m L,0.79 ug/L,418.12 ng/m L and 2335.12 ng/m L,respectively.Conclusions The expression levels of serum Vaspin,CX3 CL1 and TXNIP are different in different stages of occurrence and development of coronary heart disease.Joint detection of the above indicators is of great significance in predicting different pathological changes in patients with CHD.
