卵巢癌超声造影参数与血管新生的相关性研究

Correlation between Ultrasound Contrast Parameters and Angiogenesis in Ovarian Cancer

目的探讨与研究卵巢癌超声造影参数与血管新生的相关性。方法选择卵巢癌患者199例(恶性组)与良性卵巢肿瘤患者100例(良性组),所有患者都给予常规超声与超声造影,记录造影参数;采用病理分析检测血管新生情况并进行相关性分析。结果恶性组的病灶形态不规则、边界不清晰、内部回声不均匀、微钙化、侧方声影的发生率显著高于良性组(P<0.05)。恶性组的始增时间、峰值时间少于良性组,始增强度多于良性组,对比差异都有统计学意义(P<0.05)。恶性组病灶组织的MVD计数显著多于良性组(P<0.05)。在恶性组中,Spearman相关分析显示病灶组织的MVD计数与峰值时间呈显著性负相关(P<0.05),与始增时间、始增强度无显著相关性(P<0.05)。结论卵巢癌超声造影可为卵巢肿瘤良恶性鉴别诊断提供依据,与病理组织血管新生有显著相关性,可在术前用于评价卵巢癌血管的新生情况。

Objective To investigate the correlation between ultrasound contrast parameters and angiogenesis in ovarian cancer.Methods 199 cases of ovarian cancer were selected(malignant group)and 100 cases of benign ovarian tumors were also selected(benign group).All the patients were given conventional ultrasound and contrast enhanced ultrasound,and the parameters were recorded.The pathological angiogenesis changes were detected by pathological analysis and were given the correlation analysis.Results The incidence of irregular shape,indistinct boundary,inhomogeneous echo,microcalcification and lateral acoustic shadow in the malignant group were significantly higher than that of thebenign group(P<0.05).The initial increase time and peak time of the malignant group were less than those of the benign group,and the initial increase intensity were more than that of the benign group,compared the difference were statistically significant(P<0.05).The MVD count in malignant group were significantly higher than that of the control group(P<0.05).In the malignant group,Spearman correlation analysis showed that the MVD count of the lesion were significantly negatively correlated with the peak time(P<0.05),and had no significant correlation with the initial increase time and the initial increase intensity(P<0.05).Conclusion The contrast enhanced ultrasound can provide a basis for the differential diagnosis of benign and malignant ovarian tumors,and has a significant correlation with the neovascularization of pathological tissue.It can be used to evaluate the angiogenesis of ovarian cancer before operation.
