浅表性膀胱癌患者术后应对方法及影响因素分析

Coping Strategies and Influencing Factors of Patients with Superficial Bladder Cancer after Operation

目的分析浅表性膀胱癌患者术后应对方法及影响因素。方法随机选取浅表性膀胱癌患者224例,采用一般调查表、医学应对问卷(MCMQ)对其进行调查,然后比较224例患者的术后应对方法和全国常模,单因素分析、多重线性逐步回归分析224例患者的术后应对方法的影响因素。结果浅表性膀胱癌患者的术后MCMQ面对因子评分显著低于全国常模(P<0.05),回避因子、屈服因子评分均显著高于全国常模(P<0.05)。224例患者中,术后面对43例,回避40例,屈服29例,分别占总数的38.4%、35.7%、25.9%。不同性别、年龄、文化程度、膀胱灌注方案、付费方式患者的术后应对方法之间的差异均显著(P<0.05)。多重线性逐步回归分析显示,224例患者的术后面对的影响因素为性别、文化程度(P<0.05),回避的影响因素为文化程度、膀胱灌注方案(P<0.05),屈服的影响因素为文化程度、手术方式(P<0.05)。结论浅表性膀胱癌术后患者应对方法的影响因素多种多样,临床应该对患者的应对方法进行重点评估,然后指导并帮助患者采取积极的应对方法。

Objective To analyze the coping methods and influencing factors of patients with superficial bladder cancer after operation.Methods 224 patients with superficial bladder cancer were randomly selected.The general questionnaire and MCMQ were used to investigate the coping styles of 224 patients after operation and the national norm were compared.Single factor analysis and multiple linear stepwise regression analysis were used to analyze the influencing factors of 224 patients'coping styles after operation.Results The score of MCMQ in patients with superficial bladder cancer after operation was significantly lower than that of the national norm(P<0.05).Avoidance factor and yield factor scores were significantly higher than the national norm(P<0.05).Of the 224 patients,43 were faced with the operation,40 were avoided and 29 were yielded,accounting for 38.4%,35.7%and 25.9%of the total.There were significant differences in coping styles among patients with different gender,age,education level,bladder irrigation scheme and payment method(P<0.05).Multiple linear stepwise regression analysis showed that the influencing factors of 224 patients after operation were gender and education level(P<0.05).The influencing factors of avoidance were education level,bladder perfusion scheme(P<0.05),and the influencing factors of surrender were education level and operation method(P<0.05).Conclusion There are many factors influencing the coping styles of patients with superficial bladder cancer after operation.Clinically,the coping styles of patients should be emphatically evaluated,and then the patients should be guided and helped to adopt positive coping styles.
