腹腔镜和开腹楔形切除术治疗胃间质瘤的疗效比较

Value of laparoscopic surgery and open wedge-shaped resection for patients with gastric stromal tumor

目的比较腹腔镜和开腹楔形切除术治疗胃间质瘤患者的围术期指标、安全性及对预后的影响。方法选取2015年3月至2018年2月在安岳县第三人民医院接受治疗的胃间质瘤患者40例进行回顾性分析,其中20例采取开腹楔形切除术,设为对照组,另外20例实施腹腔镜手术,设为观察组。比较2组患者围术期指标(手术时间、术中出血量、术后排气时间、住院时间)、安全性指标(切口感染、创面出血、肠梗阻、胃瘫)和预后指标(复发率、转移率、病死率)情况。结果观察组患者手术时间、术中出血量、术后排气时间、住院时间优于对照组,差异均有统计学意义(P<0.05)。观察组组患者并发症率低于对照组,差异有统计学意义(P<0.05)。2组患者复发率、转移率比较,差异无统计学意义(P>0.05)。结论胃间质瘤患者手术治疗中,腹腔镜手术的应用能够改善患者围术期指标,提升治疗的安全性,有利于改善预后。

Objective To analyze the effect of laparoscopic surgery and open wedge-shaped resection on the perioperative indicators,safety and influence on prognosis for patients with gastric stromal tumor.Methods A total of 40 patients with gastric stromal tumor treated in Third People′s Hospital of Anyue County from March 2015 to February 2018 were selected for the study.Among them,20 patients underwent open wedge-shaped resection were set as the control group,and the other 20 patients underwent laparoscopic surgery were set as the observation group.The perioperative indicators(operation time,intraoperative blood loss,postoperative exhaust time,hospitalization time),security index(infection of incision,wound bleeding,intestinal obstruction,gastric paralysis)and prognosis index(rate of metastasis,recurrence and mortality)of the two groups were compared.Results The operation time,intraoperative blood loss,postoperative exhaust time and hospitalization time of patients in the observation group were better than those in the control group,with statistically significant differences(P<0.05).The complication rate of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in recurrence rate and metastasis rate between the two groups(P>0.05).Conclusion In the surgical treatment for patients with gastric stromal tumor,the application of laparoscopic surgery can improve the perioperative indicators and safety of treatment,which is conducive to improve the prognosis of patients.
