摘要
目的探讨PPARα通路靶向基因二肽基肽酶(DPP-4)及其基因多态性与脑动脉粥样硬化的相关性。方法选取本院神经内科门诊及住院部2017年10月至2018年8月收治的100例动脉粥样硬化性脑梗死患者为疾病组,另选取同期于本院体检的健康者100例作为正常对照组,比较两组的一般临床资料、血常规、血脂等生化指标、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)等炎性指标水平以及基因频率及等位基因频率,采用多因素Logistic回归分析脑动脉粥样硬化的危险因素。结果疾病组的收缩压、空腹血糖(FBG)、总胆固醇(TG)、甘油三酯(TC)以及低密度脂蛋白胆固醇(LDL-C)水平、IL-6 及TNF-α 水平显著高于正常对照组(P<0.05);疾病组C 等位基因携带者基因型(CC+CT)频率显著高于正常对照组,T 等位基因携带者基因型(TT+CT)频率显著低于正常对照组(P<0.05);多因素Logistic 分析结果显示,收缩压、FBG、TG、LDL-C 以及CC 基因型为脑动脉粥样硬化的重要危险因素。结论PPARα 通路靶向基因DPP4 及其基因多态性与脑动脉粥样硬化具有相关性,可能与调控炎症反应有关。
Objective To investigate the relationship between dipeptidyl peptidase(DPP-4)and polymorphism of PPAR alpha pathway targeting gene and cerebral atherosclerosis.Methods 100 patients with atherosclerotic cerebral infarction were selected as the disease group from October 2017 to August 2018 in the department of outpatient and inpatient and neurology of our hospital.Another 100 healthy people who underwent physical examination in our hospital during the same period were selected as the normal control group.The general clinical data,blood routine,blood lipid and other biochemical indicators,and leukocyte mediators of the two groups were compared.Inflammatory markers such as IL-6,TNF-alpha,gene frequency and allele frequency were measured.Risk factors of cerebral atherosclerosis were analyzed by multivariate Logistic regression.Results Systolic blood pressure,fasting blood glucose(FBG),total cholesterol(TG),triglyceride(TC),low density lipoprotein cholesterol(LDL-C),IL-6 and TNF-alpha levels in the disease group were significantly higher than those in the normal control group(P<0.05),and the frequency of C allele carrier genotype(CC+CT)in the disease group was significantly higher than that in the normal control group(P<0.05).The frequency of T allele carriers genotype(TT+CT)was significantly lower than that of the control group(P<0.05).Multivariate Logistic analysis showed that systolic blood pressure,FBG,TG,LDL-C and CC genotypes were important risk factors for cerebral atherosclerosis.Conclusion The polymorphism of DPP4 and its gene targeting PPAR alpha pathway is related to cerebral atherosclerosis and may be related to the regulation of inflammatory response.
