医疗失效模式与效应分析联合认知行为干预对甲状腺切除术患者的影响

Influence of healthcare failure mode and effect analysis combined with cognitive behavioral intervention of patients undergoing thyroidectomy

目的探讨基于医疗失效模式与效应分析(HFMEA)联合认知行为干预对甲状腺切除术患者的影响。方法选择2017年12月至2018年9月在我院行甲状腺切除术的46例患者为研究对象,随机将其等分为对照组和联合组,两组患者均给予基于HFMEA模式的护理干预,联合组同时给予认知行为干预措施。比较两组患者的护理干预效果。结果干预后两组患者的焦虑和抑郁评分较干预前均明显降低,且干预后联合组的焦虑、抑郁评分均低于对照组(P<0.05);联合组手术并发症发生率低于对照组(P<0.05)。干预后联合组QLQ-C30各维度评分均高于对照组(P<0.05)。结论基于HFMEA模式的护理联合认知行为干预应用于甲状腺切除术患者中,有助于缓解其心理应激程度,减轻术后疼痛和手术并发症的发生率,促进患者术后康复,提高生活质量。

Objective To explore the effect of healthcare failure mode and effect analysis(HFMEA)combined with cognitive behavioral intervention of patients undergoing thyroidectomy.Methods We selected 46 cases of patients who underwent thyroidectomy in our hospital from December 2017 to September 2018 as a study object.They were randomly divided into control group and combination group.Both groups were given nursing intervention of HFMEA mode.The combination group also was given cognitive behavioral interventions.The effect of nursing intervention was compared between the two groups.Results After intervention,the anxiety and depression scores of the two groups were significantly lower than those before the intervention,and the anxiety and depression scores of the combined group were lower than those of the control group(P<0.05).The incidence of complications in the combined group was lower than that in the control group(P<0.05).The each dimension scores of QLQ-C30 in the combined group were higher than those in the control group(P<0.05).Conclusion The nursing and cognitive behavioral intervention based on HFMEA mode were applied to patients with thyroidectomy,which could alleviate the psychological stress,reduce the incidence of postoperative pain and surgical complications,promote postoperative rehabilitation and improve life of quality.
