CLIA法、PA法与IFA法测定肺炎支原体的对比分析 被引量:1

摘要 目的使用全自动化学发光法(CLIA)、日本富士瑞必欧被动凝集法(PA)和间接凝集荧光法(IFA)三种方法对肺炎支原体(MP)及其分型特异性抗体进行检测,通过综合比较判断,旨在寻找一种更快捷、灵敏度高、特异性高的检测MP的方法。方法在本市各家医院留取96例疑似诊断MP感染的患者标本,分别用全自动化学发光法、PA法及IFA法检测。结果(1)CLIA法在不分型的情况下与PA法的Kappa等于0.563,P<0.05。(2)全自动CLIA检测的MP-IgG、MP-IgM同时为阳性时与PA法测试的阳性标本符合率为100%。(3)CLIA法与IFA法对MP-IgM分型抗体测试比较:Kappa值为0.81,P<0.05。(4)CLIA法检验分型抗体,当MP-IgM为阳性时,同时用PA法测得阳性为83.3%的符合率;MP-IgG为阳性时,同时用PA法测得阳性为75.0%的符合率。(5)有两例样本CLIA法检测MP-IgG大于200 au/mL,显著增高,MP-IgM阴性,PA法测得为阴性,拿到亚辉龙总部测试,结果一致,占2%比例,不具有普遍性。结论CLIA法同PA与IFA法比较具有大批量操作、定量分型、省时、特异性高、操作全自动等优点,为临床提供更全面更快捷的实验室检测信息。 Objective The detection of mycoplasma pneumoniae(MP)and its specific antibodies was conducted using three methods:the fully automatic chemiluminescence method(CLIA),the Japan Fuji Rubio passive coagulation method(PA),and the indirect condensation fluorescence method(IFA).Through comprehensive comparative judgment,the purpose is to fnd a faster,more sensitive and more specifc method to detect MP.Methods 96 cases of patients suspected to have been diagnosed with MP infection were collected in various hospitals in the city.They were examined by full automatic chemiluminescence method,PA method and IFA method.Results(1)The Kappa of CLIA method with PA method in the case of no typing is equal to 0.563 P<0.05.(2)MP-IgG,MP-IgM at the same time positive of automatic CLIA detection and PA test positive specimens of the rate of compliance is 100%.(3)Comparison of MP-IgM antibody test between CLIA method and IFA method:Kappa value is 0.81,P<0.05.(4)CLIA method test the type of antibody,when the MP-IgM is positive,at the same time measured with PA method positive 83.3%of the compliance rate;When the MP-IgG was positive,the rate of positive 75%was measured by PA method.(5)Two samples were tested by CLIA method for MP-IgG greater than 200 au/mL,signifcantly increased,MP-IgM was negative,PA method was negative,and the results were consistent when tested in Yahuilong headquarters,accounting for 2%of the total,which was not universal.Conclusion The CLIA method has the advantages of mass operation,quantitative classification,time-saving,high specificity,and automatic operation compared with the PA and IFA methods,providing more comprehensive and rapid laboratory detection information for clinical purposes.
