摘要
目的对X线、CT和MRI在早期强直性脊柱炎骶髂关节病变诊断中的应用价值进行探究。方法本次研究通过在本院2016年10月-2018年11月接受治疗的患者中进行抽样,选取了96例患者作为样本,按照检查方式,分为X线组、CT组、MRI组,对检查结果的影响表现以及分级情况进行对比分析。结果在本次研究当中,在不同检查方案下,患者的检出率存在一定的差异,X线的检出率仅仅为71.88%,CT组的检出率为87.5%,而MRI组检出率为100%,差异有统计学意义(P<0.05)。而在关节面变窄、侵蚀以及软骨肿胀等方面,X线与其他检查方式存在较大的差异;在关节面骨质囊变方面,三种检查方式的差异比较明显,但是在关节面变宽、关节面下骨质硬化方面并没有表现出明显的差异。在对早期强直性脊柱炎患者进行分级检查的时候,MRI检查在0级患者当中的检查结果与其他检查方式有着显著的差异;在Ⅰ级患者检查中,三种检查结果之间的两两相比都存在统计学的差异;Ⅱ级患者当中的X线检查与其他两种检查方式之间存在的差异有统计学意义(P<0.05)。但是在Ⅲ、Ⅳ级患者的检出率方面,三种检查手段之间差异无统计学意义(P>0.05)。结论在当前的影像学征象以及诊断当中,早期强直性脊柱炎骶髂关节病变在进行诊断的过程中,MRI检查的效果最优,而在分级检查当中,MRI在各个级别当中的检查效果都较好。
Objective To explore the application value of X-ray,CT and MRI in the diagnosis of sacroiliac joint lesions in early ankylosing spondylitis.Methods In this study,96 patients were selected as samples in the hospital from October 2016 to November 2018.According to the examination method,they were divided into the X-ray group,the CT group and the MRI group.The impact performance of the inspection results and the grading situation were compared and analyzed.Results In this study,there were some diferences in the detection rate of patients under diferent examination schemes.The detection rate of X-ray was only 71.88%,that of CT group was 87.5%,and that of MRI group was 100%.The diference was statistically significant(P<0.05).There are significant differences between X-ray and other examinations in articular surface narrowing,erosion and cartilage swelling.There are obvious differences among the three examinations in articular surface bone cyst degeneration,but there are no obvious differences in articular surface widening and subarticular bone sclerosis.In the grading examination of patients with early ankylosing spondylitis,there are significant differences between the results of MRI examination in grade 0 patients and other examinations;in gradeⅠpatients,there are statistical diferences between the two results of the three examinations;in gradeⅡpatients,there are statistically signifcant diferences between X-ray examination and other two examinations(P<0.05).But in the detection rate of gradeⅢandⅣpatients,there was no statistically significant difference between the three methods(P>0.05).Conclusion Among the current imaging signs and diagnosis,in the process of diagnosis of sacroiliac joint lesions in early ankylosing spondylitis,the effect of MRI is the best,while in the grading examination,the effect of MRI in all levels is better.
