磁共振动脉自旋标记成像技术检测急性脑梗死缺血半暗带的临床应用 被引量:5

摘要 目的:评价磁共振(MR)动脉自旋标记技术(ASL)在检测急性脑梗死缺血半暗带中的临床应用价值。方法:搜集2015年1月至2018年9月间于我院就诊的37例(男21例,女16例)具有肢体症状的单侧急性期缺血性脑梗死,回顾分析患者的常规MR及ASL表现。结果:所有患者常规MR均可见弥散加权成像(DWI)梗死区异常高信号。33例表现为梗死区低灌注,其中23例存在缺血半暗带,表现为低灌注区面积(SCBF)大于DWI异常高信号面积(SDWI)。23例存在缺血半暗带患者中预后良好17例,其余14例患者中预后良好仅4例。结论:MR-ASL序列能有效检测急性脑梗死患者缺血半暗带,可推广应用于临床诊断。 Objective:To assess the clinical applicable value of magnetic resonance(MR)arterial spin labeling(ASL)in detecting ischemic penumbra in acute cerebral infarction.Methods:From January 2015 to September 2018,37 cases(21 males and 16 females)with limb symptoms of unilateral acute ischemic cerebral infarction in our hospital were collected,and their routine MR and ASL manifestations were retrospectively analyzed.Results:All patients showed abnormally high signal in the infarct area on diffusion weighted imaging(DWI)by conventional MR.33 patients showed low perfusion in the infarct area,and 23 of them had ischemic penumbra,which showed that the area of low perfusion area(SCBF)was larger than that of DWI with abnormal high signal area(SDWI).There were 17 patients among 23 patients with ischemic penumbra had well prognosis,and only 4 patients had well prognosis among the remaining 14 patients.Conclusion:MR-ASL sequence can effectively detect the ischemic penumbra in patients with acute cerebral infarction,which can be widely used in clinical diagnosis.
