绘本游戏治疗在白血病患儿鞘内注射中的应用

Application of picture-book game therapy in intrathecal injection of leukemia children

[目的]探究绘本游戏治疗在白血病患儿进行鞘内注射操作中的应用效果。[方法]选择2017年6月—12月我科收治的诊断为急性白血病并接受正规治疗的患儿40例,针对患儿在进行鞘内注射操作时恐惧、焦虑的心理,以及抗拒、哭闹等行为,采用合适的绘本或自行设计绘本,并联系本科实际,设计绘本延伸的游戏活动,帮助患儿减轻疼痛感。[结果]将绘本游戏治疗应用于鞘内注射操作后,患儿进行鞘内注射的依从性(90%)及医生穿刺成功率(88%)明显优于实施前(40%,70%),患儿的疼痛感觉明显低于实施前,患儿家属对护理总体满意度也高于实施前,差异均有统计学意义(P<0.05)。[结论]绘本游戏治疗应用于白血病患儿的鞘内注射操作中,有助于提高患儿的穿刺成功率和依从性,缓解疼痛感。

Objective:To explore the application effect of picture-book game in intrathecal injection for children with leukemia.Methods:A total of 40 children who were diagnosed as acute leukemia and received regular treatment from June 2017 to December were selected.In view of the fear,anxiety,resistance,crying and so on in the case of intrathecal injection,a suitable picture-book or self-designed picture-book was adopted.According to the actual situation of the department,extended game activities was designed to help children relieve pain.Results:In the application of picture book therapy to intrathecal injection,the compliance of intrathecal injection(90%)and the success rate of the doctor's puncture(88%)were better than before(40%,70%).The pain feeling of the children was lower than before.The satisfaction of the family members to the nursing was better than before,the differences were statistical significant(P<0.05).Conclusion:In the application of intrathecal injection operation of children with leukemia,it was helpful to enhance the compliance of intrathecal injection,improve the success rate of puncture and relieve the pain.
