希望水平及应对方式对血液透析病人疾病感知的影响

Influence of hope level and coping style on illness perception in hemodialysis patients

[目的]探讨血液透析病人应对方式、希望对疾病感知的影响,并分析应对方式在希望和疾病感知间的中介效应。[方法]采用一般资料调查问卷、简易疾病感知问卷(BIPQ)、医学应对方式问卷(MCMQ)以及Herth希望量表(HHI)对222例血液透析病人进行横断面调查。[结果]血液病人疾病感知得分(49.31±6.16)分;病人疾病感知与希望水平、面对应对方式及回避应对方式呈负相关,与屈服应对方式呈正相关;希望水平与屈服应对方式呈负相关。分层回归显示,屈服应对方式在血液透析病人希望水平与疾病感知间起部分中介作用,中介效应为-0.198,相对中介效应为42.3%。[结论]希望水平既可以对疾病感知起到直接作用,又可以通过屈服应对方式间接作用于疾病感知。屈服应对方式介导了希望水平与疾病感知的负面效应,是积极疾病感知的危险因素。

Objective:To explore the effects of coping styles and hopes on hemodialysis patients,and to analyze the mediating effects of coping styles between hope and illness perception.Methods:A cross-sectional survey of 222 hemodialysis patients was conducted by using the General Questionnaire,the Brief Illness Perception Questionnaire(BIPQ),the Medical Coping Modes Questionnaire(MCMQ),and the Herth Hope Inder(HHI).Results:The illness perception score of hemodialysis patients was(49.31±6.16)points.Patients’illness perception was negatively correlated with hope level,avoidance,and acceptance-resignation;patients’illness perception was positively correlated with confrontation.Hope level was negatively correlated with confrontation.The stratified regression showed that confrontation played a partial mediating role between the hope level of hemodialysis patients and illness perception,with a mediating effect of-0.198 and a relative mediating effect of 42.3%.Conclusions:The hope level played a direct role in illness perception and indirectly role in disease perception through confrontation.The confrontation played mediating negative effects between hope level and illness perception,which was a risk factor for positive disease perception.
