基于流程再造和Delphi法构建医院慢性病管理护理服务标准方案

Construction of nursing standards of noninfectious chronic disease management in hospital based on process reengineering and the Delphi Method

[目的]构建医院慢性病管理护理服务标准方案,为医院慢性病管理护理服务的展开提供指导。[方法]采用头脑风暴法,对现行的医院慢性病管理护理服务流程进行增删修改,绘制出新的流程,基于新的流程制作医院慢性病管理护理服务标准方案条目,采用专家咨询法对标准方案条目进行修改,根据专家咨询结果,小组内讨论决定医院慢性病管理护理服务标准方案。[结果]初步确定了新的医院慢性病管理护理服务流程,列出30条标准条目进行了两轮专家咨询,专家对指标重要性赋值分别为3.84~4.94分、4.05~4.95分,协调系数为0.130~0.250,经χ^2检验,P均<0.01;最终确定的医院慢性病管理护理服务标准方案包括5项一级指标,18项二级指标,29项三级指标。[结论]基于流程再造和Delphi法确定的医院慢性病管理护理服务标准方案条目清晰,规范性和可操作性高,可为慢性病管理护理服务工作的展开提供依据。

Objective:To construct the nursing standard of noninfectious chronic disease(NCD)management in hospital and to provide guidance for the development of NCD nursing in the hospital.Methods:Using the brainstorming method the current NCD management and nursing service process was modified to draw a new process.The items of hospital NCD management nursing standard based on the new process.The items were revised by the experts'consultation method.The nursing standard of NCD management in the hospital was constructed,according to the results of the experts'consultation and the intergroup discussion.Results:The new nursing process of NCD management in the hospital was initially determined.Two rounds of experts'consultation were conducted on 30 standard items.The experts'assigned importance values were 3.84?4.94 points and 4.05?4.95 points,respectively;the coordination coefficient was 0.130?0.250;after Chi?square test,all P value<0.01.The final nursing standard of NCD management in the hospital included 5 first?level indicators,18 second?level indicators,and 29 specific requirements.Conclusions:The nursing standard of NCD management in hospital based on process reengineering and the Delphi method had clear items,standardization,and high operability,which could provide a basis for the development of NCD management nursing service.
