二陈汤加减治疗痰湿阻滞型椎动脉型颈椎病疗效及对脑血流参数的影响

Effect of Erchen Decoction on cervical spondylosis of vertebral artery type with syndrome of phlegm-dampness stasis and its Influence on cerebral blood flow parameters

目的观察二陈汤加减治疗痰湿阻滞型椎动脉型颈椎病的疗效及对脑血流参数的影响。方法将80例痰湿阻滞型椎动脉型颈椎病患者随机分为观察组和对照组各40例。观察组给予二陈汤加减方治疗,对照组给予西医常规治疗,2组均治疗4周。对比2组治疗前后主要临床症状和体征(眩晕、颈肩痛、头痛、心理及社会适应、日常生活及工作)、脑血流参数[颅内椎-基底动脉的舒张末期峰值血流速度(Vd)和收缩期峰值血流速度(Vp)]的变化情况,并对比2组临床总有效率和治疗期间不良反应发生情况。结果治疗后,观察组总有效率为90.0%,明显高于对照组的77.5%(P<0.05);2组治疗后眩晕、颈肩痛、头痛、心理及社会适应、日常生活及工作评分和Vd、Vp均显著升高(P均<0.05),且观察组上述各项评分及Vd、Vp均明显高于对照组(P均<0.05);2组治疗期间血常规、尿常规、肝功能、肾功能均未出现明显异常。结论二陈汤加减治疗痰湿阻滞型椎动脉型颈椎病能够快速缓解患者症状和体征,改善脑血流参数,提高椎-基底动脉舒张末期峰值血流速度和收缩期峰值血流速度,且安全性良好。

Objective It is to observe the effect of Erchen Decoction on cervical spondylosis of vertebral artery type with syndrome of phlegm-dampness stasis and its Influence on cerebral blood flow parameters.Methods Eighty patients with cervical spondylosis of vertebral artery type with syndrome of phlegm-dampness stasis were randomly divided into observation group and control group,40 cases in each group.The observation group was treated with Erchen Decoction and the control group was treated with conventional western medicine.Both groups were treated for 4 weeks.The changes of the main clinical symptoms and signs(vertigo,neck and shoulder pain,headache,psychological and social adjustment,daily life and work),cerebral blood flow parameters[end diastolic peak velocity(Vd)and peak systolic velocity(Vp)of intracranial vertebral-basal artery]before and after treatment were observed in the two groups,the total clinical effectiveness of the two groups and the occurrence of adverse reactions during treatment were compared.Results After treatment,the total effective rate of the observation group was 90.0%,which was significantly higher than 77.5%of the control group(P<0.05).After treatment,the scores of vertigo,neck and shoulder pain,headache,psychological and social adjustment,daily life and work and Vd and Vp were significantly increased(P<0.05),and the above scores and Vd and Vp were significantly higher in the observation group than those in the control group(P<0.05).No obvious abnormalities of routine examination of blood and urine,liver function and kidney during the treatment period were found.Conclusion Erchen decoction can quickly relieve the symptoms and signs,improve cerebral blood flow parameters,and increase the peak blood flow velocity and peak systolic blood flow velocity of the vertebral-basal artery with good safety in the patients with cervical spondylosis of vertebral artery type with syndrome of phlegm-dampness stasis.
