台湾地区全民健保制度公平性及其评价

Fairness and Evaluation of National Health Insurance System in Taiwan Province

低收入群体更需要健康照顾,但往往缺乏支付能力。将医疗卫生资源使用权向低收入群体倾斜,能显著提升医疗服务价值。从公平性视角,分析我国台湾地区健保制度如何更好地实现对低收入群体健康照顾,如综合实施一元化全民健保制度、全民健保保费贷款和减免制度、重大疾病低自负比例制度、山地与离岛居民特殊医疗照顾制度等。大陆地区新医改应借鉴台湾地区经验,建立基于提升低收入群体健康水平机制组合,才能经由医疗卫生服务利用均等化路径,实现“健康中国2030”战略目标。

Low-income groups need more health care,but are often lack of the ability to pay.Inclining the right to use health care resources to low-income groups can significantly improve the value of health care services.From the perspective of equity,this paper analyzes how the health insurance system in Taiwan province can better realize the health care for low-income groups,such as the comprehensive implementation of the unified universal health insurance system,the universal health insurance premium loan and exemption system,the system of low self-sufficiency for major diseases,the special medical care system for mountain and outlying islanders,etc.In order to achieve the strategic goal of"Healthy China 2030",the China's Mainland should learn from Taiwan province's experience and establish a mechanism based on improving the health care level of low-income groups.
