摘要
目的探讨起始基础胰岛素优化管理项目在糖尿病患者血糖管理中的应用效果。方法接受起始基础胰岛素治疗的100例2型糖尿病患者随机分为对照组和观察组,每组各50例,对照组出院后实施常规管理,观察组出院后实施优化管理;均随访3个月后对比两组患者治疗前后空腹血糖、餐后2 h血糖、糖化血红蛋白(HbAlc)等指标,采用糖尿病患者(成人)知识量表(ADKnowl)评估两组患者治疗前后糖尿病知识的掌握情况,记录两组患者胰岛素用量和不良反应发生情况。结果出院3个月后,两组患者空腹血糖、餐后2 h血糖、HbAlc指标及ADKnowl评分均较治疗前有显著改善,且观察组较对照组改善更显著,差异均有统计学意义(P<0.05);观察组患者基础胰岛素平均使用剂量低于对照组,不良反应发生率(12.0%)低于对照组(28.0%),差异均有统计学意义(P<0.05)。结论起始基础胰岛素治疗后实施优化管理,能够有效提升患者的血糖控制情况,提高患者的糖尿病知识掌握程度,并减少不良反应的发生。
Objective To evaluate the clinical efficacy of optimized management project on blood glucose management of diabetic patients receiving initial basic insulin therapy.Methods Firstly,100 patients of type II diabetes mellitus(DM)subjected to initial basic insulin therapy were randomly divided a control group(n=50)and an experimental group(n=50).The control group were administered with routine management of initial basic insulin therapy and 3-month follow-up while the experimental group were administered with‘optimized management’of initial basic insulin therapy and 3-month follow-up.After treatment and follow-ups,the indexes of fasting blood glucose,2-hour postprandial blood glucose and HbAlc,etc.,of the two groups as observed before and after treatment were compared.Secondly,DM patients(adult)knowledge scale(ADKnowl)was used to assess the knowledge of diabetes of two groups as documented before and after treatment.The dosage of insulin and incidence of adverse reactions were compared between the two groups.Results Both groups were found to achieve higher scores in fasting blood glucose,2-hour postprandial blood glucose,HbAlc and ADKnowl after discharge from a 3-month session of treatment as compared with those before treatment,with scores of the experimental group being even higher.The differences were statistically significant(P<0.05).The average dosage of basic insulin applied to the experimental group was lower than that applied to the control.The rate of incidence of adverse reactions of the experimental group(12.0%)was considerably lower than that of the control group(28.0%).The difference was statistically significant(P<0.05).Conclusions‘Optimizing management’of initial basic insulin can efficiently better the control of blood glucose,enhance the patients’grasp of knowledge of diabetes mellitus,and reduce the incidence of adverse reactions.
