合理情绪疗法对慢性阻塞性肺疾病患者焦虑抑郁状态的影响

Efficacy of rational-emotive therapy in treating anxiety and depression in patients with chronic obstructive pulmonary disease

目的探讨合理情绪疗法(rational-emotive therapy,RET)对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者焦虑、抑郁状态的影响。方法在呼吸内科住院治疗且合并焦虑和/或抑郁状态的COPD患者140例,随机分为观察组和对照组各70例;均在对症治疗基础上给予常规健康教育,观察组另给予RET干预。采集患者病史、肺功能、焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分、COPD评估测试量表评分(CAT)等信息,比较两组患者干预前后的焦虑、抑郁状况及生活质量。结果RET干预2周后,观察组SAS和SDS总分均显著低于对照组;在睡眠、精力、外出信心、家务活动、胸闷等方面的改善情况显著优于对照组,差异均有统计学意义(P<0.05)。结论RET能显著缓解COPD患者的焦虑抑郁情绪,改善生活质量。

Objective To evaluate the efficacy of rational-emotive therapy(RET)in treating anxiety and depression in patients with chronic obstructive pulmonary disease(COPD).Methods Firstly,140 COPD patients complicated with anxiety and/or depression admitted to the respiratory department were randomly divided into an experimental group(n=70)and a control(n=70).Secondly,both groups were subjected to a course of medical treatment and routine health education in which the experimental group were additionally treated with RET.The medical history,lung functionality,score of self-rating depression scale(SDS),score of self-rating anxiety scale(SAS)and COPD assessment test(CAT)of both groups were documented.Lastly,the states of anxiety and depression and quality of life of the two groups as observed before and after intervention were compared.Results After 2 weeks of RET intervention,the scores of SAS and SDS of the experimental were significantly lower than those of the control group(P<0.05).The experimental group were also found to do significantly better than the control in improvement on sleep,energy,self-confidence,chores and chest tightness(P<0.05).Conclusions RET can effectively reduce anxiety and depression in COPD patients and thus improve their quality of life.
