当归芍药散治疗Ⅳ期糖尿病肾病的临床效果

Clinical effect of Danggui Shaoyao San on stage Ⅳ diabetic nephropathy

目的分析当归芍药散治疗Ⅳ期糖尿病肾病(DN)患者的临床效果。方法将80例Ⅳ期DN患者随机分为观察组和对照组,每组40例。两组均给予常规治疗,观察组同时给予当归芍药散治疗。治疗前后,比较两组的尿微量白蛋白排泄率、24 h尿蛋白水平、血清肾功能指标及血脂水平,评价治疗后两组的总有效率。结果观察组的总有效率高于对照组(P<0.05)。治疗后,观察组的尿微量白蛋白排泄率、24 h尿蛋白水平低于治疗前及对照组;血清肌酐、尿素氮和尿酸水平较治疗前改善且血清肌酐、尿素氮水平低于对照组;血总胆固醇、LDL-C、HDL-C水平均较治疗前改善,且LDL-C水平低于而HDL-C水平高于对照组(均P<0.05)。两组均未发生严重不良反应。结论在常规治疗的基础上应用当归芍药散,能更好地改善Ⅳ期DN患者的临床症状及肾功能,调节血脂代谢,疗效确切,安全性较好。

Objective To analyze the clinical effect of Danggui Shaoyao San on patients with stageⅣdiabetic nephropathy(DN).Methods Eighty patients with stageⅣDN were randomly divided into observation group and control group,with 40 cases in each group.Both groups received routine therapy,and the observation group was also administered Danggui Shaoyao San.Before and after treatment,urinary microalbumin excretion rate,24-hour urinary protein level,serum renal function indicators and blood lipids levels were compared between the two groups,and total effective rate was assessed in both groups after treatment.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,the observation group had lower urinary microalbumin excretion rate and 24-hour urinary protein level as compared with the pretreatment and the control group,achieving lower serum creatinine,blood urea nitrogen and LDL-C levels and a higher HDL-C level than the control group,together with improved serum creatinine,blood urea nitrogen,uric acid,TC,LDL-C and HDL-C levels than the pretreatment(all P<0.05).No severe adverse reactions occurred in the two groups.Conclusion Administration of Danggui Shaoyao San on the basis of routine therapy can better improve clinical syndromes and renal function and regulate blood lipid metabolism in patients with stageⅣDN,obtaining a definite efficacy and favorable safety.
