腹腔镜下广泛子宫切除联合淋巴结清扫术对宫颈癌术后康复及生存质量的影响

Effect of Laparoscopic Extensive Hysterectomy Combined with Lymph Node Dissection on Postoperative Rehabilitation and Quality of Life of Cervical Cancer

目的探讨腹腔镜下广泛子宫切除联合淋巴结清扫术对宫颈癌术后康复及生存质量的影响。方法选取85例宫颈癌患者进行研究,根据不同的术式将入选者分为对照组42例和研究组43例,对照组患者于常规开腹下行广泛子宫切除联合淋巴结清扫术,研究组患者于腹腔镜下行广泛子宫切除联合淋巴结清扫术,比较两组患者的手术情况、术后康复情况、生存质量及并发症发生率。结果研究组患者的切口长度、术中出血量、阴道切除长度、术后排气时间、肠胃道恢复时间、引流管拔除时间、镇痛时间、抗生素应用时间及住院时间均显著少于对照组(P<0.05),术后3个月两组患者的生存质量评分较术前均显著升高,且研究组高于对照组(P<0.05),研究组患者并发症发生率显著低于对照组(P<0.05)。结论腹腔镜下广泛子宫切除联合淋巴结清扫术创伤性小、术后康复快,且能显著提高患者的生存质量,减少术后并发症,在宫颈癌治疗中具有重要的推广价值。

Objective To explore the effect of laparoscopic extensive hysterectomy combined with lymph node dissection on postoperative rehabilitation and quality of life of cervical cancer.Methods 85 patients with cervical cancer were selected as the subjects.According to different procedures,all patients were divided into the control group with 42 cases and the study group with 43 cases.The control group were treated with conventional open belly extensive hysterectomy combined with lymph node dissection,the study group were treated with conventional laparoscopic extensive hysterectomy combined with lymph node dissection.Surgical,postoperative recovery,quality of life,and complication rate were compared between the 2 groups.Results The incision length,intraoperative blood loss,vaginal resection length,postoperative venting time,gastrointestinal recovery time,drainage tube removal time,analgesia time,antibiotic application time and hospital stay in the study group were significantly less than those of the control group(P<0.05).3 months after surgery,The quality of life scores of the 2 groups were significantly higher than those of the preoperative group,and the study group was higher than the control group(P<0.05).The incidence of complications in the study group was significantly lower than that of the control group(P<0.05).Conclusion Laparoscopic extensive hysterectomy combined with lymph node dissection can significantly improve the quality of life and reduce postoperative complications,with small trauma and quicker postoperative recovery,which has important promotion value in the treatment of cervical cancer.
