近现代老年病发病机制学说浅述

On the theory of the pathogenesis of senile diseases in modern times

在步入老龄化社会的今天,研究老年病的发病机制尤为必要,本文总结了近现代中医老年病发病机制学说文献,概括为主虚说和主虚实说两大类,其中本虚以脾肾较显,虚实以气虚血瘀、肾虚血瘀为著,并可能夹杂气郁、风火、痰浊等其他病理产物。针对老年患者脾肾两虚、气虚血瘀、肾虚血瘀的病理特征提出老年疾病的治疗应在辨证论治的基础上,注重顾护脾肾,并适当助以疏通经络,活血化瘀之品。基于这些机理,笔者临证中以自拟“夕阳红”方(制何首乌25 g,黄酒蒸豨莶草12 g,菟丝子12 g,杜仲12 g,牛膝12 g,女贞子12 g,桑叶9 g,忍冬藤12 g,生地黄12 g,桑椹15 g,金樱子15 g,黑芝麻15 g,旱莲墨15 g,黄精15 g,丹参15 g)加减治疗各类老年常见病,疗效比较满意。

In the aging society today,it is of special importance to study the pathogenesis of geriatrics,this article summarizes traditional Chinese medicine literature on the geriatrics pathogenesis theory in modern times,which includes mainly two categories,of which the deficiency in spleen and kidney is explicit,which is accompanied with actual situation to qi deficiency and blood stasis,kidney and blood stasis,and possibly qi depression and wind,fire,phlegm turbidity and other pathological products.According to the pathologic features of spleen and kidney deficiency,qi deficiency and blood stasis,the treatment of diseases in the elderly should be based on syndrome differentiation and treatment,with attention paid to protecting the spleen and kidney,and with appropriate help to dredge the meridians,promote blood circulation and remove blood stasis.Based on the mechanism,the author prescribes Xiyanghong Formula(“夕阳红”方)(Preparation of polygonum multiflorum 25 g,steamed yellow wine siegulae 12 g,dodder 12 g,eucommia ulata 12 g,achyranthes bidentata 12 g,fructus ligustris 12 g,mulberry leaves 9 g,honeysuckle 12 g,raw rehmannia rehmanniae 12 g,mulberry 15 g,yingzi 15 g,black sesame 15 g,nasturtium glabra 15 g,huangjing 15 g,salvia miltiorrhiza 15 g)for all kinds of senile diseases,of which the curative effect is ideal.
