全身运动质量评估用于早产儿及小婴儿脑发育随访的临床研究

摘要 目的:对全身运动质量评估在早产儿和小婴儿脑发育随访研究中的作用进行研究。方法:将2016年2月-2017年2月我院接治的100例早产儿和小婴儿作为观察对象,均在扭动运动和不安运动两个阶段进行GMs评估,并随访12个月,应用格塞尔婴幼儿发展量表(Gesell)明确运动发育情况。结果:本组100例早产儿及小婴儿中,脑神经发育正常70例,脑神经发育迟缓27例,脑瘫3例;对脑发育异常预测结果,扭动阶段的敏感性为50.00%、特异性为84.33%、阳性及阴性的预测值分别为57.67%、79.68%;不安运动阶段的敏感性为13.33%、特异性为97.10%、阳性及阴性预测值分别为66.67%、72.27%;不安运动阶段异常脑瘫预测的特异性、阳性预测值最高(P<0.05)。结论:在早产儿及小婴儿脑发育随访研究中应用GMs评估有积极作用,尤其在脑瘫预测中有显著价值。 Objective:To study whole body exercise quality assessment for follow-up of brain development in premature and small infants.Method:From February 2016 to February 2017,100 cases of premature infants and small infants admitted and treated in our hospital were taken as observation subjects.All of them were evaluated by GMs in the two stages of squirming movement and restless movement,and were followed up for 12 months.Gesell infant development scale(Gesell)was applied to determine motor development.Results:In this group of 100 premature infants and small infants,there were 70 cases of normal cerebral neurodevelopment,27 cases of delayed cerebral neurodevelopment,3 cases of cerebral palsy;The sensitivity and specificity were 50.00%,84.33%,57.67%and 79.68%,respectively in the prediction of brain development abnormalities in the twist phase.The sensitivity and specificity were 13.33%,97.10%,66.67%and 72.27%,respectively in restless movement stage.The specificity and positive predictive value of abnormal cerebral palsy in restless movement stage were the highest(P<0.05).Conclusion:The application of GMs in the follow-up study of brain development of premature infants and small infants has a positive effect,it has significant value especially in the prediction of cerebral palsy.
