摘要
目的了解西安地区女性人乳头状瘤病毒(HPV)感染情况及与宫颈病变程度的关联性。方法应用实时荧光PCR方法检测2 450例女性患者分泌物中HPV基因类型并分析。另分析其中98例液基薄层细胞学(TCT)阳性患者的HPV与宫颈病变的关联性。结果2 450例患者HPV阳性率达19.7%(482/2 450);各月间的阳性率差异有统计学意义(P=0.001),各季度差异无统计学意义(P=0.477);<25岁和55岁~64岁组阳性率明显高于其他组(P<0.05);以单一感染模式为主(360例,74.7%),52型、16型、58型、39型、56型为高发类型;98例TCT阳性患者,HPV阳性率为57.1%(56/98),随病变严重程度阳性率逐渐上升;感染模式在病变分组中差异无统计学意义(P>0.05),主要致病HPV类型为16型、52型、58型、39型、56型。结论西安地区妇女HPV阳性率较高,以婚龄前和中老年女性多见,52型、16型、58型、56型、39型5种为高发类型。应加强本地区HPV预防工作,对本地区女性宫颈癌的筛查和防治工作有重要指导意义。
Objective To understand the infection of human papilloma virus(HPV)and its correlation with cervical lesions among women in Xi'an.Methods Real-time fluorescent PCR technique was used to detect HPV in 2 450 women patients,cervical exfoliated cell specimens,and then the infection of HPV was analyzed.In addition,we also analyzed the infection of HPV among 98 cases whose thinprep cytology test(TCT)results were positive.Results Among the 2 450 patients,the total infection rate was 19.7%(482/2 450);There was statistical significance on the difference among different months(P=0.001),but there was no statistical significance on the difference among different quarters(P=0.477);The infection rates between under 25 years old group and 55-64 years old group were obviously higher than other groups(P<0.05);Single subtype infection was the highest mode(360 cases,74.7%),and the most common genotype were HPV-52,HPV-16,HPV-58,HPV-39,HPV-56;Among the 98 patients,the total infection rate was 57.1%(56/98).Infection rate increased gradually with the severity of pathological changes,but there was no statistical significance on the differences in infection model(P>0.05).The main pathogenic HPV types were HPV-16,HPV-52,HPV-58,HPV-39 and HPV-56.Conclu sion In Xi'an City,HPV infection rate in women is high,the main age groups are premarital woman and middle-aged women,and common genotype are HPV-52,HPV-16,HPV-58,HPV-56,HPV-39.It is important to strengthen that prevention of HPV infection in the region and to provide screening and prevention work for the cervical cancer in the region.
