心房颤动患者血浆儿茶酚抑素水平及其对心力衰竭预后的影响

Plasma levels of Catestatin in patients with atrial fibrillation and its influence on the prognosis of heart failure

目的测定儿茶酚抑素(CST)在心房颤动患者中的表达水平,并评估其与心房颤动患者并发心力衰竭的关系。方法共入选210例患者,其中心房颤动组和窦性心律组分别为107例和103例。采用酶联免疫吸附检测法(ELISA)测定CST,运用受试者工作特征(ROC)曲线评价CST预测房颤并发心衰的价值。结果 107例房颤组平均年龄在(64.3±10.9)岁,其中有39例女性患者,占36.4%。心房颤动组患者的左房内径和BNP比对照组明显增高(P<0.01);心房颤动组患者血浆CST浓度明显高于窦性心律组患者(P<0.01)。血浆CST及CST和BNP联合指标预测心房颤动并发心力衰竭的ROC曲线下面积分别为0.649(95%CI:0.558~0.740,P<0.05)和0.680 (95%CI:0.582~0.777,P<0.05)。结论心房颤动患者较窦性心律患者的血浆CST浓度升高,CST对于房颤患者并发心力衰竭有着一定的预测价值。

Objective This paper is to measure the serum levels of catestatin(CST) in patients with atrial fibrillation(AF) and predict its effect on heart failure(HF) occurrence in patients with AF. Methods A total of 210 patients, divided into AF group(n=107) and sinus rhythm group(n=103), were selected for the study. Serum levels of CST were measured by enzyme-linked immunosorbent assay(ELISA) at baseline. Receiver operating characteristic(ROC) curve analysis was performed to predict HF occurrence in patients with AF. Results The mean age of the AF group(n=107) was(64.3±10.9) years old, and a minority of patients(36.4%) were female. The left atrial diameter and BNP of patients with atrial fibrillation were significantly higher than those of the control group(P<0.01). The plasma CST concentration in patients with atrial fibrillation was significantly higher than that in patients with sinus rhythm(P<0.01). The area under the ROC curve(AUC) of CST and combined parameters(CST and BNP) to predict HF occurrence in patients with AF was 0.649(95% CI:0.558-0.740, P<0.05) and 0.680(95% CI: 0.582-0.777, P<0.05), respectively. Conclusion The serum level of CST was significantly elevated in patients with AF, and showed a certain predictive value of HF occurrence in these patients.
