冠脉造影术中造影剂过敏一例

A case of contrast agent allergy in coronary angiography

探讨造影剂过敏的发病机制、临床症状及诊治方案。以医院一例造影术中冠脉注射碘佛醇后即出现过敏性休克的典型表现,并排除了其他可能引起过敏的因素外,确诊因碘造影剂引起的急性过敏性休克,后经及时抢救脱离生命危险的病例的诊治过程为例,对该病例前驱反应、临床症状及诊治方案进行深度分析并加以文献复习。冠状动脉造影术中造影剂过敏在临床上较少见,国内外多见于个案报道,通过该病例对冠脉造影术中造影剂过敏患者的防治提供理论基础和实践经验。

In order to investigate the pathogenesis,clinical symptoms,diagnosis and treatment of contrast agent allergy,a diagnosing process was taken as the example in our study where the angiography in coronary artery after alcohol injection of iodine caused allergic shock shown with typical signs of anaphylactic shock.In this case,an allergy was definitely caused by iodine contrast agent after excluding other factors and the patient was timely rescued and survived.Thorough analysis was made of the patient’s precursor reaction,clinical symptoms and treatment scheme.Contrast agent allergy in coronary angiography is relatively rare in clinical practice which is common in case reports at home and abroad.This case provides theoretical basis and practical experience for the prevention and treatment of patients with contrast agent allergy in coronary angiography.
