多层螺旋CT对食管癌患者术前分期诊断及预后的评估价值研究

The evaluation value of MSCT for preoperative staging and prognosis in patients with esophageal cancer

目的探讨多层螺旋电子计算机断层扫描(MSCT)对食管癌患者术前分期诊断及预后的评估价值。方法食管癌患者86例,均进行MSCT检查,并在术后随访5年。以手术病理学检查为金标准,统计MSCT进行术前T、N分期准确率,分析MSCT对食管癌患者术后1、3、5年生存期的预测价值。结果与金标准比较,MSCT检测对T 1~T 4分期诊断准确率分别为66.67%、77.78%、90.00%、95.65%,T分期总准确率为84.88%;MSCT检测对N 0~N 1分期诊断准确率分别为87.50%,90.74%,N分期总准确率为89.53%;MCST预测食管癌患者术后1、3、5年生存率的AUC值分别为0.730、0.858、0.903。结论MSCT对食管癌患者术前分期诊断准确率较高,对患者术后生存率有一定预测价值。

Objective To investigate the value of multi-slice spiral computed tomography(MSCT)for preoperative staging and prognosis of patients with esophageal cancer.Methods Eighty-six patients with esophageal cancer underwent MSCT and were followed up for 5 years after surgery.Taking surgical pathology examination as golden standard,accuracy of MSCT for preoperative T and N stage was statistically analyzed.The predictive value of MSCT for survival of the patients with esophageal cancer after 1 year,3 years and 5 years of surgery was analyzed.Results Compared with golden standard,the accuracies of MSCT detection for T 1~T 4 stages were 66.67%,77.78%,90.00%and 95.65%,respectively.The total accuracy of T stage was 84.88%.Compared with golden standard,the accuracies of MSCT detection for N 0~N 1stage were 87.50%and 90.74%,respectively.The total accuracy of N stage was 89.53%.AUC of MCST ROC analysis for predicting survival rates of patients with esophageal cancer after 1 year,3 years and 5 years of surgery were 0.730,0.858 and 0.903,respectively.Conclusion The diagnostic accuracy of MSCT is relatively higher for preoperative staging of esophageal cancer patients.MSCT has a certain predictive value for postoperative survival rate.
