双源CT灌注成像评估胃癌肿瘤血管生成及其与血清TFF3、VEGF表达的相关性研究

Dual-source CT perfusion imaging in the evaluation of tumor angiogenesis in gastric cancer and its correlation with serum TFF3 and VEGF expression

目的探讨双源CT(DSCT)灌注成像对胃癌肿瘤血管生成的临床价值及灌注成像参数与血清肿瘤新生血管标志物的关系。方法回顾性分析86例胃癌患者临床资料,以免疫组织化学法(IHC)测定的微血管密度(MVD)中位数35个/视野为界定标准,分为高密度组(MVD≥35个/视野,n=48)与低密度组(MVD<35个/视野,n=38)。比较两组DSCT灌注成像参数及血清肿瘤新生血管标志物差异,并采用Pearson相关性分析法评估各灌注成像参数与血清TFF3、VEGF之间的关系。结果高密度组血容量(BV)、血流量(BF)、表面通透性(PS)及血清TFF3、VEGF水平均高于低密度组,TTP水平低于低密度组(P<0.05),两组MTT水平比较差异无统计学意义(P>0.05)。BV、BF、PS分别与血清TFF3、VEGF呈正相关,TTP分别与血清TFF3、VEGF呈负相关(P<0.05)。结论DSCT灌注成像能对胃癌肿瘤血管生成作出较为准确的评估,且部分参数与血清TFF3、VEGF相关性良好,对评估肿瘤的生物学行为有一定参考价值。

Objective To explore the clinical value of dual-source CT(DSCT)perfusion imaging in tumor angiogenesis in gastric cancer and the relationship between perfusion imaging parameters and serum tumor angiogenesis markers.Methods The clinical data of 86 patients with gastric cancer were retrospectively analyzed.According to definition criteria that was defined as the median microvessel density(MVD)of 35/field measured by immunohistochemistry(IHC),the patients were divided into high-density group(MVD≥35/field,n=48)and low-density group(MVD<35/field,n=38).The DSCT perfusion imaging parameters and serum tumor angiogenesis markers were compared between the two groups,and the relationship between perfusion imaging parameters and serum TFF3 and VEGF was evaluated by Pearson correlation analysis.Results The BV,BF,PS and serum levels of TFF3 and VEGF in the high-density group were significantly higher than those in the low-density group while the TTP level was significantly lower than that in low-density group(all P<0.05).There was no significant difference in MTT level between the two groups(P>0.05).Pearson correlation analysis showed that BV,BF and PS were strongly positively correlated with serum TFF3 and VEGF respectively,and TTP was strongly negatively correlated with serum TFF3 and VEGF(all P<0.05).Conclusion DSCT perfusion imaging can accurately evaluate the angiogenesis of gastric cancer,and some parameters have good correlation with serum TFF3 and VEGF.Therefore,it has a certain reference value for evaluating the biological behavior of tumor.
