中医手法复位对桡骨远端骨折患者复位质量与腕关节功能恢复的影响

The effect of TCM manual reduction on reduction quality and wrist function recovery in patients with distal radius fracture

目的观察中医手法复位对桡骨远端骨折患者复位质量与腕关节功能恢复的影响,评价其疗效。方法我院收治的桡骨远端骨折患者85例,其中40例行中医手法复位小夹板外固定治疗(观察组),45例行切开复位内固定(对照组);分析两组肿胀消除时间、疼痛缓解时间、骨折愈合时间及复位治疗6个月后掌倾角、尺偏角、桡骨高度、腕关节活动度、腕关节功能Mayo评分优良率及并发症情况。结果观察组肿胀消除时间、疼痛缓解时间及骨折愈合时间均短于对照组(P<0.05)。治疗后观察组掌倾角、尺偏角及桡骨高度均小于对照组(P<0.05)。结论中医手法复位治疗桡骨远端骨折在骨折复位效果上虽不如切开复位内固定,但能够缩短肿胀消除及骨折愈合时间,且在腕关节功能恢复方面与切开复位内固定相当。

Objective To observe the effect of traditional Chinese medicine(TCM)manual reduction on reduction quality and wrist function recovery in patients with distal radius fracture,and evaluate its curative effect.Methods Eighty-five patients with distal radius fractures admitted to our hospital were divided into observation group and control group.The observation group(n=40)underwent TCM manual reduction and small splints external fixation and the control group(n=45)underwent open reduction and internal fixation.Results The swelling elimination time,pain relief time,fracture healing time,palmar and ulnar tilt,radial height,wrist range of motion and the excellent and good rate of wrist function(Mayo score)as well as complications were statistically analyzed.The swelling elimination time,pain relief time and fracture healing time of the observation group were significantly shorter than the control group(P<0.05).After treatment,the palmar and ulnar tilt and the radial height of the observation group were significantly smaller than those of the control group(P<0.05).Conclusion Although TCM manual reduction for the treatment of distal radius fracture is not as effective as open reduction and internal fixation,it can shorten the swelling elimination and fracture healing time,and it is equivalent to open reduction and internal fixation in terms of wrist function recovery.
