腰大池引流评估老年性正常压力脑积水研究

Evaluation of senile normal pressure hydrocephalus by drainage of the lumbar pool

目的分析腰大池引流评估老年性正常压力脑积水的应用效果。方法选取2016年2月—2018年8月我院神经外科收治的正常压力脑积水老年患者23例,于术前行腰大池引流评估及脑脊液引流试验。比较腰大池引流术前后临床症状改善情况、分析脑室-腹腔引流术手术效果。结果腰大池引流术术后MMSE评分高于术前,尿失禁次数及腰椎穿刺平均压力均小于术前,差异有统计学意义(P<0.05);20例患者脑室-腹腔引流术术后临床症状均得到明显改善,3例患者于术后3个月及6个月出现不适症状,调节分流泵后症状得到改善。结论腰大池引流术可作为评估老年性正常压力脑积水能否手术的重要方法,有助于提高手术安全性,促进医疗资源的合理利用。

Objective To analyze the effect of lumbar drainage on the evaluation of senile normal pressure hydrocephalus.Methods Twenty-three elderly patients with normal pressure hydrocephalus in the department of neurosurgery of our hospital from February 2016 to August 2018 were enrolled.The evaluation of lumbar drainage and cerebrospinal fluid drainage were performed before operation.To compare the improvement of clinical symptoms before and after lumbar drainage,and to analyze the effect of ventriculo-peritoneal drainage.Results The MMSE scores of the patients with lumbar drainage were higher than those before surgery,the number of urinary incontinence and the lumbar significance(P<0.05).The clinical symptoms of ventricle-peritoneal drainage were improved in 20 patients.Symptoms appeared in months and 6 months,and the symptoms improved after adjusting the shunt pump.Conclusion Lumbar drainage can be used as an important method to evaluate the operation of senile normal pressure hydrocephalus,which can improve the safety of surgery and promote the rational use of medical resources.
