右美托咪定在脓毒血症患者合并肝功能损伤中的应用 被引量:3

Application of Dexmedetomidine in Patients with Sepsis Complicated with Liver Function Damage

摘要 为了揭示右美托咪定在脓毒血症患者合并肝功能损伤中的应用效果,本研究选择我院收治的脓毒血症合并肝功能损伤的100例患者作为研究对象。将患者随机分为观察组和对照组,每组50例。对照组采用常规麻醉方法,观察组在对照组基础上加用右美托咪定。研究显示,两组患者的机械机械通气时间和ICU停留时间差异均无统计学显著性(p>0.05)。观察组治疗后的SOFA评分显著低于对照组(p<0.05)。治疗后,两组患者的4项肝功能指标水平(TBIL, AST, ALT和ALP)显著降低,而A/G水平显著升高(p<0.05)。此外,治疗后,观察组的TBIL和AST水平显著低于对照组,而A/G水平显著高于对照组(p<0.05)。两组患者治疗后的TNF-α和IL-18水平均显著降低,并且治疗后观察组的TNF-α和IL-18水平显著低于对照组(p<0.05)。本研究表明,右美托咪定可对脓毒血症合并肝功能损伤患者具有较好的肝脏保护作用,其机制可能与右美托咪定调节免疫细胞和炎症细胞因子有关。 To reveal the effect of dexmedetomidine in patients with sepsis complicated with liver function damage, 100 patients with sepsis and liver function impairment admitted to our hospital were selected as the subjects of the study. Patients were randomly divided into observation group and control group, 50 cases in each group. The control group was treated with conventional anesthesia, and the observation group was given dexmedetomidine on the basis of the control group. The study showed no significant difference in mechanical mechanical ventilation time and ICU stay time between the two groups(p>0.05). The SOFA score after treatment in the observation group was significantly lower than that in the control group(p<0.05). After treatment, the four liver function levels(TBIL, AST, ALT, and ALP) were significantly lower in the two groups, while the A/G levels were significantly h igher(p<0.05). In addition, after treatment, the TBIL and AST levels in the observation group were significantly lower than those in the control group, and the A/G level was significantly higher than the control group(p<0.05). The levels of TNF-α and IL-18 were significantly lower in the two groups after treatment, and the levels of TNF-α and IL-18 in the observation group were significantly lower than those in the control group(p<0.05). Our preliminary findings suggested that dexmedetomidine could have a better liver protection in patients with sepsis and liver damage, and its mechanism might be related to dexmedetomidine regulation of immune cells and inflammatory cytokines.
