探讨纤维蛋白原及其降解产物和D-二聚体与急性脑血管病的关系 被引量:7

The Relationship Between Fibrinogen and Its Degradation Products and D-dimer with Acute Cerebrovascular Disease

摘要 目的 探讨纤维蛋白原(FIB)及纤维蛋白原降解产物(PFDP)和D-二聚体与急性脑血管病的关系.方法 收集急性脑血管病患者300 例(脑梗死200 例、脑出血100 例)作为观察组,同期无脑血管病患者70 例作为对照组;分别比较观察组与对照组血小板、丙氨酸转氨酶、天门冬氨酸转氨酶、磷酸肌酸激酶、肌酐、FIB、D-二聚体及PFDP的差异性;观察组分别进行NIHSS评分与FIB、D-二聚体、PFDP相关性研究.结果 观察组与对照组血小板、丙氨酸转氨酶、天门冬氨酸转氨酶、磷酸肌酸激酶、肌酐差异无统计学意义(P>0.05);观察组FIB、D-二聚体以及PFDP均高于对照组,差异有统计学意义(P<0.05);观察组FIB(r=0.293)、D-二聚体(r=0.289)、PFDP(r=0.255)与NIHSS评分均呈正相关(P<0.05).结论 FIB是急性脑血管病的危险因素;FIB、D-二聚体和PFDP水平可作为预测急性脑血管病严重程度及预后的指标. Objective To explore the relationship between fibrinogen and it's degradation products and D-dimer and acute cerebrovascular disease. Methods 300 patients with acute cerebrovascular diseases (200 patients with cerebral infarction and 100 patients with cerebral hemorrhage) were enrolled in the observation group, and 70 patients without cerebrovascular diseases were enrolled in the control group during the same period. Platelet, glutamic pyruvic transaminase, glutamic oxaloacetic transaminase, phosphocreatine kinase, creatinine, FIB, D-dimer and PFDP were compared respectively between the observation group and the control group. The correlation between NIHSS scores and FIB, D-dimer and PFDP were researched in the observation group. Results There were no significant differences in platelet, glutamic pyruvic transaminase, glutamic oxaloacetic transaminase, phosphocreatine kinase and creatinine between the observation group and the control group (P>0.05). FIB, D-dimer and PFDP in the observation group were all higher than those in the control group (P<0.05). Moreover, FIB (r=0.293), D-dimer (r=0.289) and PFDF (r=0.255) were all positively correlated with NIHSS scores in the observation group (P<0.05). Conclusion FIB is a risk factor for acute cerebrovascular disease. The levels of FIB, D-dimer and PFDP all can be used as indicators to predict the severity and prognosis of acute cerebrovascular disease.
