基于聚类分析总结左明焕教授治疗结肠癌的用药规律及经验

Cluster analysis on professor Zuo Minghuan’s experience in treatment of colon cancer

目的对左明焕教授治疗结肠癌的用药规律及经验进行归纳总结。方法统计2016年12月1日-2018年3月31日间就诊于左明焕教授门诊且就诊次数≥2次的结肠癌患者,使用Excel建立数据库整理患者就诊信息转化为方剂基础数据库,对药味频数进行描述性分析,并对频数最高的33种药物进行系统聚类分析。结果97例结肠癌患者796诊次中,共用中药255种,每剂处方药味数介于11~30味,平均16.7味;累计用药频次13 355次。其中,用药次数最多者为黄芪;通过聚类分析将药物分为6类,涉及二陈丸、四神丸、健脾丸等方剂。结论左明焕教授治疗单发性结肠癌以“急则治标、缓则治本”为主要理论依据指导用药,常用的药物类型为“补虚”“清热解毒”“去湿化痰”类药物,根据结肠癌根治术后及放化疗期间患者出现的不同症状配以相应的药物,且用药精当,在防治结肠癌术后复发转移及放化疗期间并发症上有着独特的经验。

Objective To summarize the pharmaceutical rules and experiences of professor Zuo Minghuan in treatment of colon cancer.Methods The information of people with diagnosis of Colon cancer who visits the clinic of Prof.Zuo greater than or equal to 2 times from 1th December,2016-31 th March,2018 has been recorded in the EXCEL database.A descriptive analysis was used to analyze the frequency of using traditional Chinese medicine.According to the result,a system cluster analysis was used to analyze the highest frequency of 33 kinds of drugs.Results Among the 97 patients with colon cancer who came to the clinic for 796 times,255 kinds of traditional Chinese medicines were used.Each formula contains 11~30 Chinese herbs,with an average of 16.7.The cumulative medication frequency was 13,355 times.Among them,astragalus membranaceus was the most frequently used.The Chinese herbs has been divided into 6 categories by Clustering analysis,including Erchen pill,Sishen pill,Jianpi pill and other formulas.Conclusion Prof.Zuo take“symptomatic treatment in acute condition and radical treatment in chronic case”as the main principle to treat colon cancer.The commonly category of Chinese herbs is“tonify deficiency”,“cleaning heat and toxify”,“dispel dampness and phlegm”.The principle of using Chinses herbs also vary according to the symptoms during patients received colon cancer surgery,radiation and chemo therapy.Prof.Zuo has unique experience in preventing and treating postoperative recurrence and metastasis of colon cancer and complications during radiotherapy and chemotherapy.
