萍芦湿疹方治疗湿疹的临床疗效及其作用机制分析

目的观察彝族验方萍芦湿疹液和萍芦湿疹霜(统称萍芦湿疹方)治疗湿疹的临床疗效,初步探讨其作用机制。方法选择符合纳入标准的急性湿疹患者116例,分设为常规治疗的对照组和萍芦湿疹方治疗的试验组进行临床观察;复制昆明小鼠2,4-二硝基氯苯(DNCB)变应性接触性皮炎动物模型,研究萍芦湿疹方对模型小鼠耳片厚度、耳部皮肤表皮角化及真皮炎症细胞浸润,血清中细胞因子白细胞介素-1、2、4、6(interleukin-1、2、4、6,IL-1、2、4、6)和干扰素α、γ(IFN-α、IFN-γ),粒细胞-巨噬细胞集落刺激因子(GM-CSF)的影响。结果临床观察中采用萍芦湿疹方治疗58例湿疹患者,9例痊愈,36例显效,8例好转,总有效率91.38%。动物实验结果显示萍芦湿疹方可有效减轻DNCB所致小鼠变应性接触性皮炎,下调小鼠血清中的细胞因子IL-1、2、6,IFN-γ、GM-CSF浓度(P<0.05,P<0.01),增加细胞因子IL-4、TNF-α(P<0.05,P<0.01)。结论萍芦湿疹方临床及动物实验治疗湿疹有效,疗效机制可能与调节细胞因子合成,改善机体对各种湿疹诱因的反应等有关。

Objective To observe the clinical effect on eczema treated with pinglu shizhen formula of Yi nationality(pinglu shizhen liquid,pinglu shizhen cream)and explore its effect mechanism.Methods A total of 116 patients with acute eczema were collected in compliance with the inclusion criteria.A control group with routine treatment and a trial group with pinglu shizhen formula were divided to observe clinical effect.Using 2,4-DNCB,the allergic contact dermatitis models were duplicated in Kunming mice.The effect of pinglu shizhen formula was observed on ear thickness,skin keratosis of ear and infiltration of inflammatory cells in dermis as well as IL-1,2,4,6 and INF-αand IFN-αand GM-CSF.Results In clinical observation,of 58 cases with eczema treated with pinglu shizhen formula,9 cases were cured,36 cases effective remarkably and 8 cases improved.The total effective rate was 91.38%.In the experimental research,the results showed that pinglu shizhen formula effectively alleviated allergic contact dermatitis induced by DNCB,down-regulated the concentration of IL-1,2,6,IFN-αand GM-CSF and increased IL-4 and TNF-α.Conclusion Pinglu shizhen formula is effective in eczema in clinical observation and experimental research and the effect mechanism may be related to the regulation of cytokine synthesis and the improvement in various inducements of eczema.
