知柏合剂对糖尿病肾病G3a期微炎症状态的影响

Effect of Zhibai mixture on microinflammatory state in G3a stage of diabetic nephropathy

目的:观察知柏合剂对糖尿病肾病G3a期肝肾阴虚证患者的临床疗效及其对血小板/淋巴细胞(PLR)、超敏C反应蛋白(hs-CPR)等炎症因子的影响。方法:采用SAS软件随机程序,将102例DKDG3a期肝肾阴虚证患者分为两组各51例,对照组予常规西药治疗,治疗组在对照组基础上加用知柏合剂,两组均以1月为1个疗程,连续治疗3个疗程后观察血肌酐(SCr)、24h尿蛋白定量(24-UTP)、血小板/淋巴细胞比值(PLR)、超敏C反应蛋白(hs-CPR)、临床疗效的变化情况,并进行统计分析。结果:治疗后治疗组24-UTP、SCr、PLR、hs-CPR水平均明显低于对照组,差异均有统计学意义(均P<0.05)。治疗组临床总有效率明显高于对照组,两组比较有显著性差异(P<0.05)。结论:知柏合剂联合西医基础治疗能够有效降低糖尿病肾病G3a期肝肾阴虚证患者尿蛋白,降低PLR、hs-CPR,稳定肾功能,改善临床症状,从而延缓糖尿病肾病进展,提高生活质量。

Objective:To observe the clinical effect of basic western medical treatment combined with Zhibai mixture on G3a-stage diabetic nephropathy of liver-kidney yin deficiency syndrome and its effect on inflammatory factors such as platelet/lymphocyte ratio(PLR),high-sensitive C-reactive protein(hs-CRP),etc.Methods:102 cases of patients with G3a-stage diabetic nephropathy of liver-kidney yin deficiency syndrome were randomLy divided into two gtoups by SAS software,51 cases in each group.The control group was treated with conventional western medicine,while the treatment group was treated plus with Zhibai mixture on the basis of control group.For both groups,1 month as 1 course of treatment,and continuously for 3 courses of treatment.After 3 courses of treatment,the following changes were observed and statistically analyzed such as serum creatinine(Scr),24-hour urinary protein quantity(24-UTP),platelet/lymphocyte ratio(PLR),high-sensitive C-reactive protein(hs-CRP)and clinical efficiency.Results:After treatment,the levels of 24-UTP,SCr,PLR and hs-CPR in treatment group were significantly lower than those in control group,the differences were statistically significant(all P﹤0.05).The total effective rate of treatment group was significantly higher than that of control group.Conclusion:In the treatment of G3a stage diabetic nephropathy of liver-kidney yin deficiency syndrome,Zhibai mixture combined with basic western medical treatment can effectively reduce urine protein,PLR and hs-CPR,stabilize renal function,improve clinical symptoms,so as to delay the progression of diabetic nephropathy and improve the quality of life.
