达格列净联合二甲双胍治疗2型糖尿病的疗效观察

Therapeutic effect of daglitam combined with metformin on type 2 diabetes mellitus

目的:探讨达格列净联合二甲双胍治疗2型糖尿病的疗效及安全性。方法:50例2型糖尿病患者按治疗方法分为观察组(25例)和对照组(25例),均为单独使用二甲双胍治疗血糖控制不佳的患者,观察组患者加用达格列净治疗,对照组患者加用阿卡波糖治疗。比较两组患者治疗前后空腹血糖(FBG)、餐后2 h血糖(2 hPG)、HbAlc、TC、LDL-C、TG、UA、BMI及血压的变化,同时观察两组患者低血糖及泌尿生殖系统感染人次等不良反应。结果:与治疗前比较,治疗后两组患者FPG、2 hPG、HbAlc等血糖指标均降低(P<0.05),且观察组降低更明显,差异均有统计学意义(P<0.05);观察组患者BMI、SBP、DBP、TG、UA水平均较治疗前降低,差异有统计学意义(P<0.05),TC、LDL-C水平无明显下降,差异无统计学意义(P>0.05);而对照组BMI、血压、血脂、血尿酸水平无明显变化(P>0.05);两组患者观察期间不良反应发生率比较差异无统计学意义(P>0.05);且均未发生严重并发症。结论:达格列净联合二甲双胍治疗2型糖尿病具有良好的降糖效果,提高降糖达标率,兼有减低体质指数、血压及血尿酸作用,且不增加低血糖及泌尿生殖系统感染发生风险,值得临床推广应用。

Objective:To investigate the efficacy and safety of Dapagliflozin combined with metformin in the treatment of type 2 diabetes mellitus.Methods:50 patients with type 2 diabetes were selected in the hospital.According to the treatment method,there were 25 cases in the observation group and 25 cases in the control group.All of them were treated with metformin alone.The observation group was treated with Dapagliflozin,while the control group was treated with acarbose.The changes of fasting blood glucose(FBG),postprandial blood glucose(2hPG),HbAlc,TC,LDL-C,TG,UA,BMI and blood pressure were compared between the two groups before and after treatment.The adverse reactions such as hypoglycemia and urinary and reproductive system infection were also observed.Results:Compared with before treatment,the blood sugar indexes of FPG,2hPG and HbAlc in the two groups decreased after treatment(P<0.05),and the decrease in the observation group was more significant(P<0.05);the levels of BMI,SBP,DBP,TG and UA in the observation group were lower than those before treatment(P<0.05),while the levels of TC and LDL-C did not decrease significantly(P>0.05).There were no significant changes in BMI,blood pressure,blood lipid and serum uric acid levels in the control group(P>0.05);there was no significant difference in the incidence of adverse reactions between the two groups during the observation period(P>0.05);and no serious complications occurred.Conclusion:Dapagliflozin combined with metformin in the treatment of type 2 diabetes mellitus has a good hypoglycemic effect.It can improve the hypoglycemic rate,reduce body mass index,blood pressure and serum uric acid,and do not increase the risk of hypoglycemia and urinary and reproductive system infection.It is worthy of clinical application.
