PFNA内固定治疗老年股骨粗隆间骨折临床研究

Clinical study of PFNA internal fixation in the treatment of intertrochanter fracture in the elderly

目的:探讨股骨近端锁定钢板和PFNA内固定治疗老年股骨粗隆间骨折的临床效果。方法:70例老年股骨粗隆间骨折患者按平均分配方法分组,分成研究组和对照组。对照组患者接受股骨近端锁定钢板固定法进行医治,研究组患者接受PFNA内固定治疗。治疗后,对比两组患者临床指标与Harris髋关节功能评分。结果:研究组患者的手术时间和术中失血量与对照组相比,所用时间较少,失血量也较少,差异均具有统计学意义(P<0.05);而两组患者的住院时间与骨折愈合时间差异均无统计学意义(P>0.05);两组患者Harris髋关节功能评分的优良率差异无统计学意义(P>0.05)。结论:股骨近端锁定钢板和PFNA内固定治疗老年股骨粗隆间骨折的临床效果相似,可以依据患者实际情况,选择合适的固定方式。

Objective:To study and compare the clinical effects of proximal locking plate and PFNA internal fixation in the treatment of intertrochanter fracture in the elderly.Methods:70 elderly patients with intertrochanteric fracture of femur were treated in our hospital.According to the average distribution method,they were divided into study group and control group.The patients in the control group were treated with proximal locking plate fixation,and the patients in the study group were treated with PFNA internal fixation.After treatment,the clinical indexes and Harris hip joint function score were compared between the two groups.Results:compared with the control group,the operation time and intraoperative blood loss in the study group were less and the blood loss was less(P<0.05),the difference was statistically significant,but the hospitalization time and fracture healing time in the two groups were no different(P>0.05),and there was no significant difference in Harris hip joint function score between the study group and the control group(P>0.05).Conclusion:the clinical effect of proximal locking plate and PFNA internal fixation in the treatment of intertrochanter fracture of the elderly is similar,and the appropriate fixation method can be selected according to the actual situation of the patients.
