RT-3DE检查对心肌梗死合并室间隔穿孔患者诊断符合率的影响

The effect of RT-3DE examination on the diagnostic coincidence rate of patients with myocardial infarction complicated with ventricular septal perforation

目的探讨RT-3DE(实时三维超声心动图)检查对心肌梗死合并室间隔穿孔患者诊断符合率的影响。方法选取本院2016年12月至2018年7月心肌梗死合并室间隔穿孔患者41例,所有患者分别采取二维超声心动图及RT-3DE检查。统计比较二维超声心动图及RT-3DE对心肌梗死合并室间隔穿孔诊断符合率,并分析心肌梗死合并室间隔穿孔RT-3DE影像学特征。结果RT-3DE诊断符合率100.00%(41/41)高于二维超声心动图80.49%(33/41),差异具有统计学意义(P<0.05);心肌梗死合并室间隔穿孔患者经RT-3DE检查可见,左室前壁大面积心肌变薄,且运动幅度显著减弱,部分消失或呈反常搏动,室间隔连续性中断,且室间隔中断部位可见自左向右的室水平分流。结论心肌梗死合并室间隔穿孔经RT-3DE检查具有典型影像学特征,通过RT-3DE检查对疾病予以诊断可有效提高诊断符合率,降低漏诊风险,避免延误患者最佳治疗时机。

Objective To investigate the effect of RT-3DE(real-time three-dimensional echocardiography)on the diagnostic coincidence rate of patients with myocardial infarction complicated with ventricular septal perforation.Methods Forty-one patients with myocardial infarction complicated with interventricular septum perforation were enrolled in our hospital from December 2016 to July 2018.All patients underwent two-dimensional echocardiography and RT-3DE.The diagnostic accuracy of two-dimensional echocardiography and RT-3DE in the diagnosis of myocardial infarction combined with ventricular septal perforation was analyzed.The imaging features of myocardial infarction combined with interventricular septal perforation RT-3DE were analyzed.Results The diagnostic coincidence rate of RT-3DE was 100.00%(41/41)higher than that of two-dimensional echocardiography 80.49%(33/41),the difference was significant(P<0.05).In patients with myocardial infarction complicated with ventricular septal perforation,it can be seen by RT-3DE that the left ventricular anterior wall has a large area of myocardial thinning,and the amplitude of motion is significantly weakened,partially disappeared or abnormally beaten,the ventricular septal continuity is interrupted,and the ventricular septal interruption is visible.The left to right chamber is horizontally diverted.Conclusion Myocardial infarction combined with ventricular septal perforation has typical imaging features by RT-3DE.Diagnosis of disease by RT-3DE can effectively improve the diagnostic coincidence rate,reduce the risk of missed diagnosis,and avoid delaying the optimal treatment time.
