2型糖尿病患者绝经后骨质疏松危险因素的Logistic回归分析

Logistic regression analysis for the risk factors of postmenopausal osteoporosis in patients with type 2 diabetes mellitus

目的研究2型糖尿病患者绝经后骨质疏松的危险因素并予以Logistic回归分析。方法将2016年2月至2018年2月收治的绝经后发生骨质疏松的2型糖尿病患者50例作为观察对象,为研究组。另取同期接受治疗的绝经后未发生骨质疏松的2型糖尿病患者50例为参照组。分别比较2组患者年龄及血脂、血压指标水平,采用双能X线吸收法测量并比较2组患者骨密度水平,并作Logistic回归分析。结果研究组患者年龄、三酯甘油、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇、收缩压水平分别为(57.32±2.04)岁、(2.35±1.01)mmol/L、(3.38±1.03)mmol/L、(1.35±0.30)mmol/L、(142.28±12.08)mm Hg,比参照组的(49.27±1.87)岁、(1.96±0.90)mmol/L、(2.62±1.04)mmol/L、(1.15±0.19)mmol/L、(133.58±10.94)mm Hg高,且舒张压水平为(73.18±6.69)mm Hg,比参照组的(80.86±9.49)mm Hg低,差异有统计学意义(P<0.05)。研究组患者双股骨密度、腰椎1~4骨密度分别为(0.93±0.05)g/cm^2、(0.92±0.14)g/cm^2,比参照组的(1.92±0.14)g/cm^2、(1.24±0.20)g/cm^2低,差异有统计学意义(P<0.05)。经多因素Logistic回归分析可得:年龄、三酯甘油、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇、收缩压均是2型糖尿病患者绝经后骨质疏松的独立危险因素(P<0.05)。结论2型糖尿病患者绝经后骨质疏松的发生与年龄、三酯甘油、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇、收缩压密切相关,值得临床重点关注。

Objective To investigate the risk factors of postmenopausal osteoporosis in patients with type 2 diabetes mellitus by means of Logistic regression analysis.Methods A total of 50 patients with postmenopausal osteoporosis and with type 2 diabetes mellitus who were admitted and treated in our hospital from February 2016 to February 2018 were enrolled as research group,at the same time,the other 50 patients with type 2 diabetes mellitus without postmenopausal osteoporosis were enrolled as control group.The patient’s age,blood lipid levels,blood pressure levels were observed and compared between the two groups.Moreover the bone mineral density levels were measured by dual-energy X-ray absorption method,and Logistic regression analysis was performed.Results There were significant differences in the patient’s age,the levels of triglyceride,total cholesterol,LDL cholesterol,HDL cholesterol and systolic blood pressure,and diastolic blood pressure levels between the two groups(P<0.05).In research group,the double femur density and the bone density of the lumbar spine 1~4 were(0.93±0.05)g/cm 2 and(0.92±0.14)g/cm 2,respectively,which were significantly lower than those[(1.92±0.14)g/cm 2 and(1.24±0.20)g/cm 2]in control group(P<0.05).Multivariate Logistic regression analysis showed that patient’s age,triglyceride,total cholesterol,LDL cholesterol,HDL cholesterol and systolic blood pressure were independent risk factors for postmenopausal osteoporosis in patients with type 2 diabetes mellitus(P<0.05).Conclusion The pathogenesis of postmenopausal osteoporosis in patients with type 2 diabetes mellitus is closely related to patient’s age,triglyceride,total cholesterol,LDL cholesterol,HDL cholesterol and systolic blood pressure.
