血清IL-17、IL-22、IL-26与脓毒症相关肾病损伤的相关性分析 被引量:9

摘要 目的探讨血清白细胞介素(IL)-17、IL-22、IL-26与脓毒症相关肾病损伤(AKI)的相关性。方法选取2016年9月至2018年9月期间该院收治的146例脓毒症患者作为研究对象,根据是否合并AKI分为单纯脓毒症(n=81)和脓毒症合并AKI组(n=65)。比较各组件基本生化指标及血清IL-17、IL-22、IL-26水平,并进行Pearson相关性分析和经logistic回归分析。结果脓毒症合并AKI组Scr、BUN、eGFR、UA、APACHEⅡ评分、CRP、PCT、IL-17、IL-22、IL-26均显著高于对照组(P<0.05)。经logistic回归分析,IL-17、IL-22、IL-26是脓毒症患者相关肾病损伤的影响因素。经过Pearson相关性分析,IL-17、IL-22、IL-26与脓毒症合并AKI患者Scr、BUN、eGFR、UA均呈正相关(P<0.05)。脓毒症合并AKI患者好转组血清IL-17、IL-22、IL-26水平显著低于恶化组(P<0.05)。结论IL-17、IL-22、IL-26是脓毒症相关肾病损伤的独立影响因素,且可以评估预后。 Objective To investigate the relationship between serum interleukin-17(IL-17),interleukin-22(IL-22),interleukin-26(IL-26)and acute renal injury(AKI)in septic related nephropathy.Methods From September 2016 to September 2018,146 patients with sepsis were selected.The patients were divided into sepsis group(n=81)and sepsis combined with AKI group(n=65)according to whether they were associated with AKI.The basic biochemical indexes and the serum levels of IL-17,IL-22,IL-26 were compared,and Pearson correlation analysis and logistic regression analysis were carried out.Results The Scr,BUN,eGFR,UA,APACHE II score,IL-17,IL-22,IL-26 in sepsis with AKI group were significantly higher than those in control group(P<0.05).By logistic regression analysis,IL-17,IL-22,IL-26 were found to be the influential factors of nephrotic injury in septic patients.By Pearson correlation analysis,there was a positive correlation between IL-17,IL-22,IL-26 and Scr,BUN,eGFR,UA in septic patients with AKI(P<0.05).The serum levels of IL-17,IL-22,IL-26 in the patients with sepsis complicated with AKI were significantly lower than those in the patients with exacerbation(P<0.05).Conclusion IL-17,IL-22,IL-26 are independent risk factors for septic nephropathy injury,and they can be used to evaluate the prognosis.
