甲状腺相关眼病血清sICAM-1、sVCAM-1水平变化及与病情关系分析

摘要 目的观察甲状腺相关眼病(TAO)患者血清可溶性细胞间黏附分子1(sICAM-1)及可溶性血管细胞黏附分子1(sVCAM-1)表达情况,并与病情进行相关性分析。方法将该院2016年8月至2018年3月收治的150例TAO患者纳入研究,依据临床活动性评分(CAS)分为活动期组、非活动期组,按病情严重程度分为轻度组、中重度组、极重度组,并选取同期80例健康体检者作为对照组,比较各组一般资料及血清sICAM-1、sVCAM-1水平;分析sICAM-1、sVCAM-1水平与TAO病情严重程度的相关性。结果活动期组血清sICAM-1与sVCAM-1显著高于非活动期组及对照组,对照组与非活动期组比较差异无统计学意义(P>0.05)。不同病情严重程度组的活动期患者血清sICAM-1与sVCAM-1显著高于组内活动期患者与对照组(P<0.05),极重度组、中重度组、轻度组的水平依次降低,差异有统计学意义(P<0.05)。非活动期患者病情程度与血清sICAM-1、sVCAM-1无显著相关性(P>0.05);活动期患者病情程度与血清sICAM-1、sVCAM-1呈正相关(P<0.05)。结论活动期患者病情越严重,血清sICAM-1、sVCAM-1表达水平越高,而非活动期患者无此特点,可指导TAO临床诊治。 Objective To observe the expression of soluble intercellular adhesion molecule-1(sICAM-1)and soluble vascular cell adhesion molecule-1(sVCAM-1)in serum of patients with thyroid associated ophthalmopathy(TAO),and analyze the correlation with the condition.Methods A total of 150 patients with TAO admitted to the hospital from August 2016 to March 2018 were enrolled in the study.According to the clinical activity score(CAS),they were divided into active group and inactive group.According to the severity,they were divided into mild group,moderate group and severe group.Meanwhile,80 healthy persons were selected as the control group.The general data,serum concentrations of sICAM-1 and sVCAM-1 were compared among the groups and the correlation of sICAM-1 and sVCAM-1 concentrations with the severity of TAO was analyzed.Results Serum concentrations of sICAM-1 and sVCAM-1 in the active group were significantly higher than those in the inactive group and the control group(P<0.05).However,there was no significant difference between the control group and the inactive group(P>0.05).Concentrations of sICAM-1 and sVCAM-1 in patients in active stage were significantly higher than those in inactive stage in all groups of different severity degrees and control group(P<0.05).The concentrations decreased in the order of extremely severe group,moderate to severe group,mild group(P<0.05).There was no significant correlation between serum sICAM-1 and sVCAM-1 concentrations and the severity of disease in inactive patients(P>0.05),while the concentrations were positively correlated with the degree of desease severity in active patients(P<0.05).Conclusion The more serious the condition,the higher expression levels of serum sICAM-1 and sVCAM-1 in active stage.However,there is no such characteristic in inactive stage,which can be used to guide clinical diagnosis and treatment of TAO.
