《医宗金鉴》海桐皮汤临床研究概况

Research Overview of Clinical Application of Haitongpi Decoction from “Yi Zong Jin Jian”

海桐皮汤出自清朝御制医学著作《医宗金鉴》,可用于治疗一切跌打损伤及疼痛症状,目前在临床中已得到广泛应用。本文通过对近五年中国知网、万方数据库相关研究及文献进行检索分析,阐述了海桐皮汤在骨折病及术后并发症、退行性骨关节病及其他疾病的临床应用概况。研究发现该方在骨折病及术后并发症上,海桐皮汤对桡骨远端骨折、髌骨骨折、椎体骨折、桡骨小头骨折、掌指骨骨折、Pilon骨折等骨折病的保守治疗或手术治疗后具有消肿止痛、促进骨折愈合、改善功能活动并减少术后引起的关节僵硬、肢体肿胀等并发症的作用;在治疗退行性骨关节病上,该方主要用于治疗膝骨关节炎、腰椎间盘突出症、颈椎病、肩周炎及足跟痛等疾病引起的疼痛和功能障碍;该方还可用于下肢静脉曲张性溃疡、丹毒并发的肢体肿胀、脑卒中引起的肩手综合征等疾病的治疗。该文结合目前该方的临床研究概况,提出目前研究中存在的问题,为进一步探究海桐皮汤临床疗效和机理,及为挖掘和传承方药在清宫正骨的应用经验方面提供了思路和方法。

Haitongpi decoction comes from the imperial medical works of the Qing dynasty called“Yi Zong Jin Jian”,which is specially designed to treat injuries and pains.At present,it has been widely used in clinical practice.Through the search and analysis of related research and literature on CNKI and Wanfang database in the past five years,this article reviewed the clinical application in the fields of bone fracture and complications,degenerative osteopathic and other diseases.The study found that for the conservative treatment or surgical treatment of distal radius fracture,patella fracture,vertebral fracture,radial small head fracture,metacarpal and phalanx fracture,Pilon fracture and other fracture diseases,Haitongpi decoction has the effect of reducing swelling and pain,promoting fracture healing,improving functional activities and reducing postoperative joint stiffness,limb swelling and other complications.What’s more,it can be used to treat pain and dysfunction caused by knee osteoarthritis,lumbar disc herniation,cervical spondylosis,periarthritis of shoulder and heel pain and other diseases.Besides,it can also be used for the treatment of lower extremity varicose ulcer,erysipelas complicated with limb swelling,stroke caused shoulder hand syndrome caused by stroke.This article also proposed some problems about current research on the clinical application of Haitongpi decoction,which provides ideas to further explore clinical efficacy and mechanism of Haitongpi decoction,and explore and inherit the application experience of prescriptions in the Qing dynasty bone-setting.
